低周波与针康法治疗脑卒中后肩关节半脱位的疗效比较

【摘要】：目的：本研究通过比较低周波与针康法治疗脑卒中后肩关节半脱位的临床疗效，为治疗脑卒中后肩关节半脱位提供一个更有效的、易被患者接受的康复治疗方法。材料与方法：采用随机表将符合诊断和纳入标准的60例患者分为低周波组（30例）和针康组（30例），观察各组治疗前后采用视觉模拟评分法（VAS）评分、神经功能缺损程度评分标准中上肢肌力评分及肩关节半脱位疗效判定标准评定各组的临床疗效。结果： 1.在VAS评分方面，两组治疗前后比较有显著性差异（P0.05），治疗后两组组间比较有显著性差异（P0.05）.。 2.在患侧上肢肌力评分方面，两组治疗前后比较有显著性差异（P0.05），治疗后两组组间比较无显著性差异（P0.05）。 3.治疗后综合疗效比较，低周波组显效17例，有效10例，无效3例，总有效率90.00%；针康组显效19例，有效9例，，无效2例，总有效率83.33%；总有效率比较无显著差异（P0.05）。结论： 1.低周波治疗组和针康治疗组在治疗中风后肩关节半脱位方面疗效相似。 2.低周波治疗和针康组治疗组治疗中风后肩关节半脱位，均能改善患侧上肢肌力评分，促进肩关节半脱位的康复。 3.低周波治疗组和针康治疗组，治疗中风后肩关节半脱位都能使肩峰和肱骨头之间的间距缩短，达到关节复位消除症状的目的。 4.低周波治疗组和针康治疗组均能改善、消除肩痛，但低周波治疗组的止痛效果优于针康组。
[Abstract]:Objective: to compare the clinical effect of low Zhou Bo and acupuncture on shoulder subluxation after stroke, and to provide a more effective treatment for shoulder subluxation after stroke. A method of rehabilitation that is easily accepted by a patient. Materials and methods: 60 patients who met the criteria of diagnosis and inclusion were randomly divided into low Zhou Bo group (30 cases) and acupuncture group (30 cases). Before and after treatment, the visual analogue score (VAS), the upper limb muscle strength score and the evaluation standard of shoulder subluxation were used to evaluate the clinical efficacy of each group. Results: 1. In terms of VAS score, there was a significant difference between the two groups before and after treatment (P 0.05), and after treatment there was a significant difference between the two groups (P 0.05). 2. In the score of muscle strength of the affected upper limb, there was significant difference between the two groups before and after treatment (P 0.05), but there was no significant difference between the two groups after treatment (P 0.05). 3.Compared with the comprehensive curative effect after treatment, 17 cases in low Zhou Bo group were effective, 10 cases were effective, 3 cases were ineffective, the total effective rate was 90.005%. In the acupuncture group, 19 cases were effective, 9 cases were effective, 2 cases were ineffective, and the total effective rate was 83.33%. There was no significant difference in total effective rate (P 0.05). Conclusion: 1. The curative effect of low Zhou Bo treatment group and acupuncture Kang group in treating shoulder subluxation after stroke is similar. 2. The treatment of lower Zhou Bo and acupuncture group can improve the muscle strength score of upper limb and promote the recovery of shoulder subluxation after stroke. 3. Treatment of shoulder subluxation with low Zhou Bo and acupuncture therapy can shorten the distance between acromion and humerus head, and achieve the purpose of joint reduction and elimination of symptoms. 4. Both the low Zhou Bo group and the acupuncture group can improve and eliminate the shoulder pain, but the analgesic effect of the low Zhou Bo group is better than that of the acupuncture Kang group.
