健康成人底丘脑核的MRI定位及其与年龄相关性研究

【摘要】:目的:利用3.0T MRI对底丘脑核进行多序列及多方位扫描,寻找显示底丘脑核的最佳扫描参数及扫描方位;评价正常脑老化过程中底丘脑核的形态变化,旨在探索底丘脑核的生理老化规律,为底丘脑核相关疾病的研究奠定基础。 方法:首先选取3名健康右利手志愿者,男1名,,女2名,年龄20~79岁,行MRI T1、T2、FLAIR常规检查,并采用T2-3D-DRIVE CLEAR、T2*、SWI序列分别进行轴位、冠状位及矢状位三个方位的扫描,得出最佳扫描参数及扫描方位。选取134名健康右利手志愿者,男63名,女71名,年龄20~79岁,平均年龄49岁,中位年龄30岁,并按性别、年龄分组,进行垂直于AC-PC线的T2-3D-DRIVE CLEAR冠状位扫描,显示出两侧核团的最大截面,手工绘制感兴趣区,测量两侧底丘脑核的最大长径、短径及最大截面积。应用SPSS13.0软件进行统计分析。采用成组t检验、配对t检验等比较底丘脑核不同侧别、性别、年龄组间各统计指标的差异,探索底丘脑核的形态与年龄的相关关系。 结果: 1在应用飞利浦公司Achieva3.0T超导磁共振设备条件下,经过T1、FLAIR、SWI、T2*、T2几种最常用且具有代表性的序列进行筛选和对比观察得出显示底丘脑核的MRI最佳序列为T2,鉴于底丘脑核的解剖学特点,进而选择T2-3D-DRIVE CLEAR。 比较观察包括轴位、矢状位、冠状位三个常用扫描方位对核团的显示情况,得出底丘脑核最理想的显示方位为垂直于AC-PC线的冠状位。 2对底丘脑核于垂直于AC-PC线的冠状位最大截面形态进行归纳总结,得出底丘脑核于冠状位呈现三种形态类型:双凸透镜形、羊角形、柳叶形。 3底丘脑核冠状位扫描所得两侧最大截面积、最大长径及最大短径三个结构参数与性别、侧别、不同年龄组、各组之间的统计学结果如下: 底丘脑核的冠状位最大截面积、最大长径及最大短径男女之间均不存在差异(P0.05)。 底丘脑核的两侧冠状位最大截面积、最大长径及最大短径均不存在侧别差异(P0.05)。 底丘脑核的两侧冠状位最大截面积、最大长径及最大短径不同年龄组间两两比较,得出结果为组间均无差异(P>0.05) 结论: 1底丘脑核的MRI最佳扫描序列为T2序列;最理想的显示方位为垂直于AC-PC线的冠状位。 2底丘脑核于垂直于AC-PC线的冠状位呈现三种形态类型:双凸透镜形、羊角形、柳叶形。 3底丘脑核冠状位最大截面积、最大长径及最大短径值不存在性别差异;底丘脑核冠状位最大截面积、最大长径及最大短径值不存在侧别差异;底丘脑核的冠状位最大截面积、最大长径及最大短径随年龄的增长不发生变化。
[Abstract]:Objective: to evaluate the morphologic changes of subthalamic nucleus during normal brain aging by using 3.0T MRI to scan the subthalamic nucleus in order to find out the best scanning parameters and scanning orientation of the subthalamic nucleus. The purpose of this study was to explore the physiological aging law of subthalamic nucleus and to lay a foundation for the study of subthalamic nucleus-related diseases. Methods: three healthy right-handedness volunteers, one male and two female, aged 20 to 79 years old, were selected for routine MRI T1T2FLAIR examination. The axial, coronal and sagittal positions were scanned by T2-3D-DRIVE clear T2FLAIR sequence. The optimal scanning parameters and scanning orientation were obtained. 134 healthy right-handedness volunteers were selected, 63 males and 71 females, aged 2079 years, with an average age of 49 years, with a median age of 30 years, and were grouped according to sex and age. T2-3D-DRIVE CLEAR coronal scanning perpendicular to the AC-PC line showed the maximum cross-section of the bilateral nuclei, manually plotted the region of interest, and measured the maximum length and diameter of the bilateral subthalamic nuclei. Short diameter and maximum cross-sectional area were analyzed by SPSS13.0 software. The difference of statistical indexes among different sides, sex and age groups of subthalamic nucleus was compared by group t test and paired t test. To explore the correlation between the shape of subthalamic nucleus and age. Results:. 1 under the condition of Achieva3.0T superconducting magnetic resonance equipment of Philips Company, through screening and comparing several most commonly used and representative sequences of T _ (1) FLAIRI _ (SWI) T _ (2) T _ (2), it was found that the best sequence of MRI in the subthalamic nucleus was T _ (2), in view of the anatomical characteristics of the subthalamic nucleus. Then T2-3D-DRIVE clear. By comparing and observing the display of nuclei in axial, sagittal and coronal positions, it is concluded that the optimal display orientation of the subthalamic nucleus is perpendicular to the AC-PC line. 2 the maximum sectional shape of the subthalamic nucleus in the coronal position perpendicular to the AC-PC line was summarized, and three morphological types of the subthalamic nucleus in the coronal position were obtained: double convex lens, goat horn and willow leaf. 3The maximum cross sectional area, maximum length diameter and maximum short diameter of the subthalamic nucleus were obtained by coronal scanning of the subthalamic nucleus. The three structural parameters, sex, side and age group, were as follows:. There was no significant difference in coronal cross sectional area, maximum length diameter and maximum short diameter of subthalamic nucleus between male and female. The maximum coronal cross sectional area, the maximum length and the maximum short diameter of the subthalamic nucleus were not different from each other (P 0.05). The maximum coronal cross sectional area, maximum length diameter and maximum short diameter of subthalamic nucleus were compared between different age groups. The results showed that there was no difference between the two groups (P > 0.05). Conclusion:. 1the best scanning sequence of MRI in subthalamic nucleus was T2 sequence, and the optimal display orientation was coronal position perpendicular to AC-PC line. 2 the coronal position of the subthalamic nucleus perpendicular to the AC-PC line showed three morphological types: double convex lens, goat horn and willow leaf. (3) there was no gender difference in the maximum cross-sectional area, the maximum length and the maximum short diameter of the subthalamic nucleus, the maximum cross-sectional area, the maximum length and the maximum short diameter of the subthalamic nucleus were not different from those in the coronal position of the subthalamic nucleus, and the coronal maximum cross-sectional area of the subthalamic nucleus was not different from that of the subthalamic nucleus. The maximum length diameter and the maximum short diameter do not change with the increase of age.
