昆山市2001~2012年心脑血管疾病死亡率分布及疾病负担分析

【摘要】:目的:了解昆山市2001~2012年户籍居民心脑血管疾病死亡的分布特征及其对居民健康的危害程度,为进一步制定心脑血管疾病防治的策略提供科学依据。 方法:收集昆山市2001~2012年户籍居民心脑血管疾病的死亡资料,以ICD-10进行分类编码。采用SPSS16.0软件分析心脑血管疾病死亡率、标化死亡率、年龄别死亡率、死亡构成比、死亡性别比、潜在减寿年数(PYLL)、标化减寿年数(SPYLL)、减寿率和平均减寿年数(AYLL)等指标。 结果:1.2001~2012年,,昆山市累计报告心脑血管疾病死亡病例13254例,其中男性6517例,女性6737例。2001~2012年,昆山市户籍人口心血管疾病死亡率总体呈波动性变化趋势,脑血管疾病死亡率总体呈波动性下降趋势,但是,心脑血管疾病总体死亡率一直维持在较高水平;心脑血管死亡率随年龄增长呈几何级数增高 2.2001~2012年,昆山市心血管疾病年均死亡率为65.81/10万(其中男性为63.53/10万、女性为68.09/10万),标化死亡率为34.01/10万(男性为33.89/10万、女性为34.31/10万),男女2001~2012年心血管疾病死亡率比较,经X2检验,P0.05,差异有统计学意义。脑血管疾病年均死亡率为100.75/10万(其中男性为100.58/10万、女性为100.92/10万),标化死亡率为53.76/10万(男性为54.78/10万、女性为52.92/10万),男女2001~2012年脑血管疾病死亡率比较,经X2检验,P=0.880,差异无统计学意义。 3.2001~2012年,心血管疾病前5位死因顺位排序依次为:肺源性心脏病、其他冠心病、急性心肌梗死、高血压性心脏病、慢性风湿性心脏病。 4.2001~2012年,心脑血管疾病死亡率随年龄的增长而上升,心脑血管死亡病例主要集中在45-64岁组和65岁以上年龄组,心、脑血管疾病45岁以上年龄组死亡分别占全部死亡的96.48%和98.32%,尤以65岁以上年龄组死亡人数最多,分别占87.89%和86.59%。死亡率至85岁~组达到高峰,心血管死亡率为2674.58/l0万,脑血管死亡率为2536.82/l0万。 5.女性心血管疾病死亡率高于男性,差异有统计学意义;而脑血管疾病经统计,男女差异无统计学意义。 6.2001-2012年昆山市男性居民心血管疾病、脑血管疾病潜在减寿年数,潜在减寿率(PYLL率)、标化PYLL(SPYLL)、标化PYLL率(SPYLL率)和平均减寿年数等指标均高于女性。在心血管疾病中,急性心肌梗死所造成的疾病负担最重,PYLL和PYLL率分别为4136人年、0.520‰。 结论:心脑血管疾病已成为昆山市户籍居民的主要死亡原因,全社会必须重视心脑血管疾病的预防。同时,应重点加强对60岁以上这部分年龄组人群心脑血管疾病的控制和预防。
[Abstract]:Objective: to investigate the distribution of cardiovascular and cerebrovascular diseases in residents of Kunshan City from 2001 to 2012 and to provide scientific basis for the prevention and treatment of cardiovascular and cerebrovascular diseases. Methods: the death data of cardiovascular and cerebrovascular diseases in residents of Kunshan City from 2001 to 2012 were collected and classified and coded by ICD-10. The mortality rate of cardiovascular and cerebrovascular diseases, standardized mortality, age-specific mortality and death composition ratio were analyzed by SPSS16.0 software. The sex ratio of death, the number of years of potential life lost (PYLLL), the standardized number of years of life lost (SPYLLL), the rate of life reduction and the average number of years of life lost (AYLLL) were measured. Results from 2001 to 2012, 13254 death cases of cardiovascular and cerebrovascular diseases were reported in Kunshan City, including 6517 males and 6737 females. The mortality rate of cardiovascular diseases in Kunshan population showed a fluctuating trend. The mortality rate of cerebrovascular diseases is fluctuating, but the overall mortality rate of cardiovascular and cerebrovascular diseases has been maintained at a relatively high level, and the mortality rate of cardiovascular and cerebrovascular diseases increases in geometric progression with the increase of age. From 2001 to 2012, the average annual mortality rate of cardiovascular disease in Kunshan City was 65.81 / 100 million (63.53% / 100 000 for males, 680.09% / 100,000 for females, 34.01% / 100,000 for females (33.89% / 100,000 for males and 34.31 / 10 000 for females), and a comparison of cardiovascular mortality rates between 2001 and 2012 for males and females. From 2001 to 2012, the first five causes of death of cardiovascular disease ranked in order: pulmonary heart disease, other coronary heart disease, acute myocardial infarction, hypertension heart disease, chronic rheumatic heart disease. 4. From 2001 to 2012, the mortality rate of cardiovascular and cerebrovascular diseases increased with the increase of age. Cerebrovascular diseases over 45 years old group accounted for 96.48% and 98.32% of all deaths, especially over 65 years old group, accounted for 87.89% and 86.599.The mortality reached the peak in 85 years old group, and the cardiovascular death rate was 10 000. The cerebrovascular death rate was 2536.82 / 10 million. 5. The mortality rate of cardiovascular disease in female was higher than that in male, the difference was statistically significant, but there was no significant difference between male and female in cerebrovascular disease. 6. The number of years of potential life lost and the rate of potential life loss of male residents in Kunshan City from 2001 to 2012 were higher than those of women.
