前列腺绿激光汽化术治疗良性前列腺梗阻伴帕金森病的疗效

【摘要】：目的研究经尿道前列腺绿激光汽化术(PVP)治疗良性前列腺梗阻(BPO)伴帕金森病(PD)的疗效及安全性。方法选择2008年7月至2016年5月收治的BPO伴PD患者20例,65~75岁,平均(69.85±3.12)岁,平均国际前列腺症状评分(IPSS)(23.40±5.89)分,平均最大尿流率(Qmax)(5.75±1.71)m L/s。患者均出现排尿困难症状并确诊为BPO伴PD,均排除合并多系统萎缩病(MSA),采用配对t检验进行组间比较,评估行PVP手术的疗效。结果 20例手术均顺利完成,平均手术时间(66.3±13.10)min,术后3个月患者平均IPSS(7.10±2.59)分,Qmax(14.03±4.40)m L/s,均较术前明显改善。4例患者术后出现急迫性尿失禁,经过盆底肌训练后好转。结论 PVP是BPO伴PD患者解除尿路梗阻及改善症状的一种安全有效的治疗方法。
[Abstract]:Objective to study the efficacy and safety of transurethral green laser vaporization of prostate (PVP) in the treatment of benign prostatic obstruction (BPO) with Parkinson's disease (PD). Methods Twenty patients with BPO with PD from July 2008 to May 2016 were selected. The mean age was 69.85 卤3.12 years old, the mean international prostatic symptom score was 23.40 卤5.89, and the mean maximum urinary flow rate was 5.75 卤1.71 mL / s. All the patients had dysuria symptoms and were diagnosed as BPO with PD.MSAs were excluded from patients with multiple system atrophy and compared with each other by paired t test to evaluate the effect of PVP operation. Results all the 20 cases were successfully completed, the average operation time was 66.3 卤13.10 min, and the average IPSS(7.10 卤2.59) score was 14.03 卤4.40 m / L / s in 3 months after operation, which was significantly better than that before operation in 4 patients with acute urinary incontinence, and improved after pelvic floor muscle training. Conclusion PVP is a safe and effective method for relieving urinary tract obstruction and improving symptoms in patients with BPO and PD.
