帕金森病伴抑郁的静息态fMRI研究

【摘要】:目的:通过观察帕金森病伴抑郁患者的脑血氧水平依赖信号(BOLD)的区域一致性(Regional homogeneity ReHo)和功能连通性的改变,以揭示帕金森病伴抑郁患者的神经活动特点及其病理生理机制。方法:纳入41例帕金森病患者(Parkinson’s disease PD)和25例正常对照(normal control NC)。所有患者进行汉密尔顿抑郁量表(HAMD)和自评抑郁量表(SDS)测定,进一步将患者分成两组:帕金森病伴抑郁组(PD patients with depression PDD)和帕金森病不伴抑郁组(non-depressed PD patients nD-PD)。l、采用静息fMRI区域一致性作为测量指标,进行单因素方差分析(ANOVA)比较三组间ReHo值的差异。2、将ReHo值有显著差异的脑区作为种子点,采用功能连接(functional connectivity,FC)方法,对比三组间的全脑功能网络的改变。结果:与nD-PD组患者相比,,PDD组提示在左侧额中回和右侧额下回的区域一致性增高,而在左侧杏仁核和双侧舌回降低。功能连接分析显示伴抑郁症状的患者相比不伴抑郁的患者前额叶边缘系统的功能连接减低,而在前额叶和舌回的功能连接增强。结论:PDD患者情绪调节网络的脑功能有明显改变和受损。多个脑区神经活动异常的可能是导致帕金森病伴抑郁的病理生理机制原因。
[Abstract]:Aim: to investigate the regional consistency and functional connectivity of cerebral oxygen level dependent signal (bod) in patients with Parkinson's disease (PD) and depression in order to reveal the characteristics of neural activity and its pathophysiological mechanism in PD patients with depression. Methods: 41 patients with Parkinson's disease (PD) and 25 normal controls (normal control NCV) were enrolled. All patients were measured with Hamilton Depression scale (Hamd) and self rating Depression scale (SDS). The patients were further divided into two groups: PD patients with depression PDD) in Parkinson's disease with depression group and non-depressed PD patients nD-PDD. L in Parkinson's disease with depression group. Resting fMRI region consistency was used as a measure index. Univariate ANOVA (ANOVA) was used to compare the differences of ReHo between the three groups. The brain regions with significant differences in ReHo values were used as seed points, and the functional connectivity (FCs) method was used to compare the changes of the whole brain functional networks among the three groups. Results: compared with nD-PD group, the regional consistency in left middle frontal gyrus and right inferior frontal gyrus was increased, but in left amygdala and bilateral lingual gyrus decreased. Functional junction analysis showed that the functional connections of the prefrontal limbic system in patients with depression were lower than those without depression, but the functional connections in the prefrontal lobe and lingual gyrus were enhanced. Conclusion the brain function of the emotional regulatory network in patients with 10-PDD was significantly changed and impaired. Abnormal neural activity in multiple brain regions may be the pathophysiological mechanism of Parkinson's disease with depression.
