酸冰刺激疗法与神经肌肉电刺激疗法联合应用对脑卒中后吞咽障碍的改善效果观察

【摘要】:目的观察酸冰刺激疗法与神经肌肉电刺激疗法联合应用对脑卒中后吞咽障碍的改善效果。方法116例脑卒中后吞咽功能障碍患者随机分为观察组、对照组各58例,两组均采取常规治疗及神经肌肉电刺激疗法治疗,观察组在此基础上加用酸冰刺激疗法治疗,每个疗程20 d,持续治疗2个疗程。分别采用洼田饮水试验、才藤荣—吞咽功能障碍评价法评价患者吞咽功能,另通过观察甘油三酯(TG)、血清白蛋白(ALB)、总胆固醇(TC)、BMI、三头肌皮褶厚度(TSF)评价患者营养状况。结果观察组洼田饮水试验评价治愈0例、显效30例、有效25例、无效3例、总有效率为94.83%,对照组分别为0、20、26、12例和79.31%,两组总有效率比较,P0.05;观察组治疗前后才藤荣—吞咽功能障碍分级分别为(3.8±0.8)、(5.2±0.7)级,对照组分别为(3.7±0.6)、(4.6±0.7)级,同组治疗前后比较,两组治疗后比较,P均0.05。观察组治疗前TG、ALB、TC、BMI、TSF分别为(1.32±0.42)mmol/L、ALB(33.53±2.43)g/L、(4.13±0.24)mmol/L、(21.43±0.4)kg/m2、(14.53±2.19)mm,治疗后分别为(1.72±0.32)mmol/L、(39.73±2.66)g/L、(4.63±0.84)mmol/L、(23.40±0.60)kg/m2、(19.64±1.85)mm,治疗前后比较,P均0.05;对照组治疗前TG、ALB、TC、BMI、TSF分别为(1.29±0.39)mmol/L、(33.69±2.38)g/L、(3.98±0.33)mmol/L、(20.98±0.53)kg/m2、(13.68±2.23)mm,治疗后分别为(1.53±0.51)mmol/L、(36.54±2.38)g/L、(4.42±0.25)mmol/L、(22.40±0.50)kg/m2、(16.29±2.25)mm,治疗前后ALB、TC、TSF比较,P均0.05;两组治疗后TG、ALB、TC、BMI、TSF比较,P均0.05。结论酸冰刺激疗法联合神经肌肉电刺激疗法治疗脑卒中后吞咽障碍疗效确切,且进一步改善了患者的营养状况。
[Abstract]:Objective to observe the effect of acid-ice stimulation combined with neuromuscular electrical stimulation on dysphagia after stroke. Methods 116 patients with dysphagia after stroke were randomly divided into observation group (n = 58) and control group (n = 58). Each course of 20 days, continuous treatment of 2 courses. The patients' swallowing function was evaluated by the test of drinking water in depressions, and the nutritional status of patients was evaluated by observing triglyceride (TGG), serum albumin (ALBN), total cholesterol (TC) and triceps skin fold thickness (TSFs) in order to evaluate the patients' swallowing function, and to evaluate the nutritional status of the patients by observing triglyceride (TG), serum albumin (ALB), total cholesterol (TC), and triceps skin fold thickness (TSF). Results in the observation group, 0 cases were cured, 30 cases were effective, 25 cases were effective and 3 cases were ineffective. The total effective rate of the two groups was 94.83, and that of the control group was 0.20 and 260.12 and 79.31, respectively. The total effective rate of the two groups was higher than that of the control group (P 0.05), the scores of caitongrong and dysphagia in the observation group were 3.8 卤0.810, 5.2 卤0.7) and those in the control group were 3.7 卤0.64.6 卤0.7, respectively, before and after treatment, and compared before and after treatment.
