轻型卒中后排便障碍危险因素分析

【摘要】：目的观察轻型缺血性脑卒中患者急性期排便障碍的发生率和影响因素,并探讨相关危险因素对患者预后的影响。方法回顾性分析274例首发轻型卒中患者的临床资料,根据是否存在卒中后排便障碍将其分为排便障碍组与非排便障碍组,对可能影响卒中后排便障碍的因素进行单因素、多因素logistic回归分析,并分析这些因素对排便障碍患者预后的影响。结果 274例轻型卒中患者发生排便障碍74例,发生率为27.0%,单因素分析显示基线NIHSS评分等4个因素为轻型卒中后排便障碍的危险因素,多因素分析显示女性、高龄、糖尿病、基线NIHSS评分为卒中后排便障碍的独立危险因素;伴排便障碍的轻型卒中患者3个月改良Rankin量表(mRS)差异明显(P0.05),基线NIHSS评分可预测轻型卒中后排便障碍患者的预后。结论轻型卒中患者急性期排便障碍可能增加患者不良预后的风险,女性、高龄、糖尿病、神经功能缺损严重更容易发生卒中后排便障碍,值得临床工作者重视。
[Abstract]:Objective to observe the incidence and influencing factors of defecation disorder in mild ischemic stroke patients and to explore the influence of related risk factors on prognosis. Methods the clinical data of 274 patients with first-onset mild stroke were retrospectively analyzed. According to the existence of poststroke defecation disorder, the patients were divided into defecation disorder group and non-defecation disorder group. Multivariate logistic regression analysis was used to analyze the influence of these factors on the prognosis of patients with defecation disorder. Results 74 cases (27.0%) of defecation disorder occurred in 274 patients with mild stroke. Univariate analysis showed that baseline NIHSS score was the risk factor of defecation disorder after mild stroke, and multivariate analysis showed that female, old age, diabetes mellitus. Baseline NIHSS score was an independent risk factor for defecation disorder after stroke, and the modified Rankin scale mRS3 months was significantly different in mild stroke patients with defecation disorder (P 0.05). Baseline NIHSS score could predict the prognosis of patients with defecation disorder after mild stroke. Conclusion Acute defecation disorder in mild stroke patients may increase the risk of poor prognosis. Women, old age, diabetes mellitus and neurological impairment are more likely to occur post-stroke defecation disorder, which deserves attention of clinical workers.
