注射用尤瑞克林对脑梗死后的炎症反应和神经功能缺损的影响

【摘要】:目的:越来越多的证据显示,注射用尤瑞克林可以通过改善脑梗死区域的血液灌注从而使病人获益,该药物可以舒张血管、促进血管再生,本实验旨在探讨该药物对于人体内炎症因子的调节作用,并研究该药物对神经功能缺损的疗效。方法:本实验共收集74名就诊于河北医科大学第二医院的首次发病的脑梗死患者,依据是否应用注射用尤瑞克林,分为:对照组、尤瑞克林组。其中,对照组共30人,给予神经内科基础治疗,尤瑞克林组共44人,在基础治疗之上加用注射用尤瑞克林静脉滴注。通过监测血清中白细胞介素-6(interleukin-6,IL-6)和肿瘤坏死因子(tumor necrosis factor alpha,TNF-α)的浓度变化,记录美国国立卫生研究院卒中量表(the National Institutes of Health Stroke Scale,NIHSS)和简明精神状态量表(the MiniMental State Examination,MMSE)的评分变化,来探究该药物的作用,以利于指导临床用药。结果:依据是否应用注射用尤瑞克林,将74名患者分为:对照组、尤瑞克林组,可以观察到:用药10天后,与对照组相比,尤瑞克林组的IL-6、TNF-α降低程度更大,NIHSS评分改善程度更大。另外,依据入院MMSE评分,可将上述74名患者重新分为:认知缺损组、无认知缺损组,可以观察到:认知缺损组的IL-6呈下降趋势,而无认知缺损组的IL-6呈上升趋势。结论:注射用尤瑞克林能够降低IL-6、TNF-α,改善神经功能缺损,该药物能够通过降低急性期脑梗死的炎症反应来使患者获益。另外,炎症因子IL-6可能在认知缺损的发生、发展过程中扮演了重要角色。
[Abstract]:Objective: there is growing evidence that eucrine for injection benefits patients by improving blood perfusion in the infarcted area, and that the drug relaxes blood vessels and promotes vascular regeneration. The aim of this study was to investigate the regulatory effect of the drug on inflammatory factors in human body and to study the effect of the drug on neurological impairment. Methods: a total of 74 patients with cerebral infarction who were first diagnosed in the second Hospital of Hebei Medical University were selected and divided into control group and eurekline group according to whether or not to use eurekline for injection. 30 patients in the control group were treated with basic neurology, 44 patients in the Urekline group were treated with intravenous drip of Urekline for injection in addition to the basic treatment. The changes of serum levels of interleukin-6 (IL-6) and tumor necrosis factor alpha-TNF- 伪 (TNF- 伪) were measured to record the changes of the National Institutes of Health Stroke scale (NIHSS) and the MiniMental State scale of mental state (MMSE) in the National Institutes of Health (NIH). To explore the role of the drug in order to guide clinical use. Results: 74 patients were divided into control group and eurexine group according to whether or not they were used for injection. It was observed that IL-6 TNF- 伪 decreased more significantly than that of control group after 10 days of medication, and the improvement of NIHSS score was greater. In addition, according to the MMSE score of admission, the above 74 patients can be re-divided into cognitive defect group, no cognitive defect group, it can be observed that the IL-6 of cognitive defect group showed a downward trend, but no cognitive defect group IL-6 showed an upward trend. Conclusion: Urethrin for injection can reduce IL-6 TNF- 伪 and improve nerve function defect. This drug can benefit patients by reducing inflammatory reaction of acute cerebral infarction. In addition, inflammatory factor IL-6 may play an important role in the occurrence and development of cognitive impairment.
