我国六省市农村地区脑卒中高危人群筛查数据分析

【摘要】:目的:通过对我国六省市农村地区≥40岁人群脑卒中高危人群筛查数据分析,探讨我国农村地区脑卒中患病情况及相关危险因素,为制定农村脑卒中防治策略,开展脑卒中危险因素干预提供依据。方法:本文所用数据来自国家人口健康科学数据共享平台中国卒中数据中心,是国家卫生计生委新医改重大专项《脑卒中高危人群筛查与防治工程》数据资源。数据来自2012年我国六省市农村地区≥40岁384272例筛查对象。研究上述六省市农村地区居民脑卒中患病、危险分层及主要危险因素分布情况。结果:1、人群脑卒中患病情况:北京市、河南省、山东省、四川省、山西省、陕西省六省市农村地区40岁及以上人群中脑卒中患者共8196例,检出率为2. 13%。其中男性4274例,检出率为2. 39%;女性3922例,检出率为1.95%。男性高于女性,差异有统计学意义,P0. 001。2、脑卒中患病地域分析:在北京市、河南省、山东省、四川省、山西省、陕西省六省市农村地区384272例筛查人群中,脑卒中检出率河南最高,为3. 56%,此后依次为北京市2.95%,山东省2.73%,山西省2.33%,陕西省1.29%,四川省0.63%。筛查人群的脑卒中患病存在地区差异。3、脑卒中患病年龄段分析:8196例脑卒中患者中,40~64岁的患者有4387例,占53.53%。随着年龄增大,脑卒中检出率增高。55岁以上人群脑卒中检出率上升到接近人群脑卒中检出率,以后年龄组检出率呈大幅度上升,各年龄组间脑卒中患病差异有统计学意义,P0. 001。4、脑卒中风险评估:我国六省市农村地区384272例筛查人群中,脑卒中低危人群 277928 例(72. 33%),中危人群 51622 例(13. 43%),高危人群 54722 例(14. 24%)。5、人群脑卒中危险因素分析:我国六省市农村地区≥40岁人群中脑卒中危险因素血脂异常暴露率38. 47%最高,其次依次是吸烟(21.49%)、高血压(20. 77%)、明显超重或肥胖(17. 03%)、体育锻炼少(16. 64%)、脑卒中家族史(7.00%)、糖尿病(4.08%)、房颤或瓣膜性心脏病(2.21%)。男性脑卒中危险因素暴露率前几位依次为:吸烟(43.83%)、血脂异常(37. 44%)、高血压(18.46%)、体育锻炼少(15. 21%)、明显超重或肥胖(13.59%)。女性脑卒中危险因素暴露率排前几位的依次为:血脂异常(39. 37%)、高血压(22. 78%)、明显超重或肥胖(20. 02%)。吸烟的暴露率则为男性明显高于女性,而高血压、血脂异常、明显超重或肥胖、体育锻炼少、有脑卒中家族史、糖尿病、房颤或瓣膜性心脏病等的暴露率均为女性高于男性,差异均有统计学意义(P均0.001)。结论:1、我国六省市农村地区≥40岁人群中脑卒中患者检出率为2.13%。其中男性检出率2. 39%,高于女性(1.95%)。2、我国六省市农村地区≥40岁人群中脑卒中检出率河南最高,为3.56%,其后依次为北京市2. 95%,山东省2. 73%,山西省2. 33%,陕西省1.29%,四川省0. 63%。3、我国六省市农村地区≥40岁人群脑卒中患者中,40-64岁患者占53.53%。严重影响农村劳动力人群的健康和生活质量。农村中年人脑卒中筛查与防控应引起高度重视。4、我国六省市农村地区≥40岁筛查人群中脑卒中低危个体占72. 33%,中危个体占13. 43%,高危个体占14. 24%。5、我国六省市农村地区≥40岁人群中脑卒中危险因素血脂异常暴露率38.47%最高,其后依次是吸烟、高血压、明显超重或肥胖、体育锻炼少等。
[Abstract]:Objective: To explore the incidence of stroke and related risk factors in rural areas of our country by analyzing the screening data of the high risk population of cerebral apoplexy in the rural areas of six provinces and cities in China, and to provide the basis for the prevention and treatment of stroke risk factors in rural areas. Methods: the data used in this paper are from the national population health department. The data sharing platform of Chinese stroke data center is the data resource of the National Health Planning Commission's major new medical reform project, the screening and prevention project of high risk population for stroke. The data came from the screening objects of 384272 cases in rural areas of six provinces and cities over 40 years old in 2012. The risk stratification and the risk of the risk stratification in the rural areas of the six provinces and cities of the above-mentioned provinces and cities were studied.
