经颅多普勒超声对正常健康成人Willis环侧支循环的检测

【摘要】:目的 :应用经颅多普勒超声(TCD)评价正常健康成人Willis环侧支循环效果。方法 :选择99例正常健康成人,通过压迫同侧颈总动脉试验,应用TCD观察各组压迫前、后的大脑中动脉(MCA)收缩峰血流速度,血流速度平均值和舒张末血流速度及各期代偿率并进行左、右侧比较。结果:压颈前MCA收缩期血流速度左、右侧分别为(102.1±13.0)cm/s,(101.0±12.9)cm/s,压颈后分别为(47.4±12.4)cm/s,(47.5±11.8)cm/s,左、右侧代偿率分别为0.47±0.12,0.47±0.11。各指标左、右侧比较无显著性差异(P0.05);压颈前MCA平均血流速度左、右侧分别为(65.3±8.6)cm/s,(64.8±8.9)cm/s,压颈后分别为(33.7±8.4)cm/s,(33.6±8.5)cm/s,左、右侧代偿率分别为0.52±0.13,0.52±0.12。各指标左、右侧比较无显著性差异(P0.05);压颈前MCA舒张末期血流速度左、右侧分别为(47.0±6.9)cm/s,(46.8±7.7)cm/s,压颈后分别为(27.2±6.8)cm/s,(26.7±7.1)cm/s,左、右侧代偿率分别为0.58±0.15,0.57±0.15。各指标左、右侧比较无显著性差异(P0.05)。收缩峰期与舒张末期代偿率比较有显著性差异(P0.01),其余各期间代偿率比较无显著性差异(P0.05)。结论:TCD能实时评价颅内动脉侧支代偿效果,且左、右代偿效果无显著性差异,舒张末期的代偿效果明显高于收缩峰期。
[Abstract]:Objective: to evaluate the collateral circulation of Willis circle in healthy adults by transcranial Doppler ultrasound (TCD). Methods: TCD was used to observe the peak systolic velocity of middle cerebral artery (MCA) before and after compression in 99 healthy adults. Mean blood flow velocity, end diastolic velocity and compensatory rate were compared between left and right. Results: the systolic velocity of MCA was (102.1 卤13.0) cm / s on the right side, (101.0 卤12.9) cm / s on the right side, (47.4 卤12.4) cm / s, (47.5 卤11.8) cm / s after cervical compression, and the compensatory rates on the left and right sides were 0.47 卤0.120.47 卤0.11 respectively. The mean blood flow velocity of MCA before cervical compression was (65.3 卤8.6) cm / s, (64.8 卤8.9) cm / s, (33.7 卤8.4) cm / s, (33.6 卤8.5) cm / s, respectively, and the compensatory rates of left and right sides were 0.52 卤0.13 卤0.52 卤0.122.The mean blood flow velocity of MCA before cervical compression was (65.3 卤8.6) cm / s, (64.8 卤8.9) cm / s, (33.6 卤8.5) cm / s, respectively. There was no significant difference between the left and the right side (P0.05), the left and right end diastolic velocity of MCA were (47.0 卤6.9) cm / s, (46.8 卤7.7) cm / s, (27.2 卤6.8) cm / s, (26.7 卤7.1) / cm / s, respectively, and the compensatory rates of left and right sides were 0.58 卤0.15 卤0.57 卤0.15 respectively. There was no significant difference between the left and the right side (P0.05). There was significant difference between peak systolic period and end-diastolic compensatory rate (P0.01), but there was no significant difference in other periods (P0.05). ConclusionTwo TCD can evaluate the compensatory effect of intracranial artery collateral in real time, and there is no significant difference between left and right. The compensatory effect at the end of diastole is obviously higher than that in peak systolic period.
