急性一氧化碳中毒后迟发性脑病大鼠海马区HIF-1α及VEGF蛋白动态表达的实验研究

【摘要】:目的:本研究通过建立DEACMP大鼠模型,应用Morris水迷宫实验、组织病理学、免疫组织化学染色法、TUNEL细胞凋亡检测法等技术检测大鼠行为学和病理形态学变化,探讨HIF-1α及VEGF在相应动物模型中的表达变化,确定其在DEACMP发病机制中的作用,为进一步研究DEACMP提供新的思路和方法,为临床治疗提供实验依据。方法:将78只健康雄性成年SD大鼠(SPF级)随机分为三组:空白对照组(BC组)18只、空气对照组(AC组)30只、急性一氧化碳中毒迟发性脑病组(CO组)30只。其中CO组采用腹腔注射法建立急性一氧化碳中毒迟发性脑病大鼠模型,AC组采用腹腔注射法注射等量空气,BC组不进行任何手术(腹腔注射)操作。按中毒后1d、3d、7d、14d、21d、28d六个时间点将三个组各分为6个亚组,其中CO组和AC组每个亚组5只大鼠,BC组每个亚组3只大鼠。各组大鼠在造模前通过Morris水迷宫实验测试记忆力、定向力等认知功能,7d、14d、21d、28d时间点亚组的大鼠在处死取脑前也进行Morris水迷宫实验以测试认知功能改变。各组大鼠的脑组织切片均应用HE染色法观察大鼠海马区病理变化,应用免疫组化染色法检测其海马区HIF-1α、VEGF蛋白的表达,用TUNEL细胞凋亡检测法检测其海马区锥体细胞凋亡情况。运用SPSS17.0软件对检测结果进行统计学分析。结果:1.DEACMP大鼠模型的建立:(1)CO组大鼠腹腔注射CO气体后出现明显的中毒反应。表现为染毒后5~15min陆续出现烦躁、多动,行为表现轻度活跃;染毒20min后大鼠开始出现拒食少动,抖动,呼吸急促,呼吸幅度增大,口唇鼻黏膜和下腹部、肢端皮肤呈现明显的樱桃红色,及行动迟缓,步态异常等。部分大鼠出现躁狂、撞笼、抽搐、角弓反张,继而四肢瘫软、昏迷。给予通风、保暖,约36.7%大鼠在染毒24h内死亡,存活大鼠约8小时后完全苏醒,观察至染毒后第28天,部分大鼠出现反应偏迟钝,进食量减少,活动减少等表现,其余大鼠日常行为活动未见明显异常。(2)AC组大鼠腹腔注射等量空气后未见明显行为学异常,AC组大鼠无死亡。2.Morris水迷宫实验结果:(1)定位航行实验:三个组平均逃避潜伏期在染毒前和染毒后第7d无显著差别,无统计学意义(P0.05);染毒后第14d、21d、28d时,CO组平均逃避潜伏期均长于BC、AC组,有统计学差异(P0.05),BC组与AC组间无统计学差异(P0.05)。(2)空间探索实验:1)第IV象限(平台所在象限)游泳时间/总时间:染毒前、染毒后第14d三个组第IV象限游泳时间/总时间无统计学差异(P0.05);染毒后第7d、21d、28d时CO组比AC、BC组小,有统计学差异(P0.05);BC组、AC组在各时间点上无统计学差异(P0.05);CO组随时间变化呈降低趋势。2)穿越平台次数:各时间点,三个组经过平台次数无统计学差异(P0.05);各组内,,不同时间点经过平台次数无统计学差异(P0.05);28d时CO组有低于其余组的趋势;CO组随时间变化,经过平台次数有降低趋势。3.脑组织病理形态学改变:HE染色标本上,(1)各时间点的CO组大鼠脑内有广泛的病理改变,海马CA1区出现大量不同程度地神经元变性坏死。主要表现为神经元胞体肿胀、皱缩,细胞数目略减少,细胞变形呈梭型或三角形,细胞层变薄、细胞间隙扩大,细胞核固缩、核仁模糊、核周出现空晕,胞质与胞核界限不清,细胞结构紊乱,在变性坏死的神经元周围可见不同程度地胶质细胞增生浸润。(染毒后第21d亚组大鼠脑切片中还可以见到坏死神经元被吸收后所留下的龛影,周围亦可见到胶质细胞的踪迹。)(2)BC与各时间点的AC组大鼠海马区的神经元大小基本正常,核膜完整,核仁清晰,胞核呈淡蓝色,着色均匀,细胞器完整。其中,海马区CA1区有两到三层锥体细胞,排列整齐致密,胞核饱满、边界清晰、尼氏小体丰富,核仁深染、清晰。4.HIF-1α及VEGF蛋白的免疫组化染色表达:(1)HIF-1α表达:HIF-1α在BC组及AC组中表达少,CO组在染毒后第1d表达明显升高,第3d达到高峰,然后逐渐降低,至28d时仍有较高表达,CO组HIF-1α表达量在各时间点均高于BC组及AC组,有统计学差异(P0.01)。(2)VEGF表达:VEGF在BC组及AC组中表达少;CO组除第14d外,其余各时间点上VEGF的表达量均高于BC和AC组,有统计学差异(P0.05);CO组VEGF表达量从染毒后第1d开始迅速升高,在第3d达最高峰后逐渐降低,至第21d时再次显著升高形成次高峰,继而又持续降低,至第28d时VEGF表达量接近第1天水平,表现为增高→降低→增高→降低的动态变化趋势。5.TUNEL细胞凋亡检测结果:CO组于染毒后第1d时检测出大脑海马CA1区有少量神经细胞凋亡,第3d时开始增多,第7~14天达到高峰(第7天为最高峰),第28d时仍有少量凋亡,凋亡指数较BC组及AC组明显增高,有统计学差异(P0.01)。结论:1.采用改良式多次间隔腹腔注射CO法制作大鼠DEACMP模型,设备要求低,操作简便易掌握,干扰因素少,CO用量少、精确,并且与人体DEACMP的病理生理变化过程相近,是研究DEACMP较为理想的一种动物模型。2.Morris水迷宫实验通过定位航行训练和空间探索测试以明确大鼠学习记忆能力和空间位置记忆能力变化,能客观评价大鼠认知行为功能,是初步判定DEACMP大鼠模型是否成功建立的一种较为可靠的依据。3.急性一氧化碳中毒后HIF-1α表达的增高可促进VEGF的表达,启动细胞凋亡信号传导通路,从而促进细胞凋亡,参与了DEACMP的发病。

After 20 minutes of exposure , the rats began to show signs of antifeedant , shaking , shortness of breath , increased respiratory amplitude , lip nasal mucosa and lower abdomen , with obvious signs of cherry red and slow gait and abnormal gait .
The average escape latency of CO group was longer than that of BC and AC groups at 14d , 21d , and 28d after exposure . There was no statistical difference between BC group and AC group ( P0.05 ) .
At the 7th , 21st , and 28th day after exposure , the CO group was smaller than AC and BC group ( P0.05 ) .
There was no statistical difference between the BC group and the AC group at all time points ( P0.05 ) .
( 2 ) There was no statistical difference between the three groups ( P0.05 ) .
There was no statistical difference in the number of stages in different time points ( P0.05 ) .
At 28 d , the CO group tended to be lower than that of the rest groups .
Immunohistochemical staining of HIF - 1伪 and VEGF protein was observed in the CA1 region of hippocampus . ( 2 ) VEGF expression : VEGF was less expressed in BC group and AC group ;
The expression of VEGF was higher in CO group than in BC and AC group except for the 14th day ( P0.05 ) .
Conclusion : 1 . The expression level of VEGF in the CA1 region of the brain increased at the first day after exposure to the first day . The results showed that the expression of VEGF in the CA1 region of the brain increased at the first day after exposure to the first day . The results showed that the expression of HIF - 1伪 in the hippocampus of rats was higher than that of the group BC and AC .
