腔隙性脑梗死患者经颅多普勒血流参数与神经损伤及炎症反应程度、Hcy代谢的相关性

【摘要】:目的:研究腔隙性脑梗死患者经颅多普勒血流参数与神经损伤及炎症反应程度、Hcy代谢的相关性。方法:选择在我院确诊为腔隙性脑梗死(LI)的88例患者作为LI组,另取同期体检的100例健康志愿者作为对照组。采用经颅多普勒超声测定大脑中动脉以及基底动脉的血流参数,采集血清并测定神经损伤分子、炎症反应指标、Hcy代谢指标。结果:LI组大脑中动脉的MFV、PI以及基底动脉的MFV、PI均显著高于对照组;LI组血清中NSE、S100B、UCH-L1、ox-LDL、MMP-9、IL-18、sCD40L、Hcy的含量显著高于对照组且与大脑中动脉、基底动脉的MFV、PI呈正相关,叶酸、维生素B12的含量显著低于对照组且与大脑中动脉、基底动脉的MFV、PI呈负相关。结论:腔隙性脑梗死患者大脑中动脉以及基底动脉的血流加快、搏动增强,颅内动脉血流参数的改变与神经损伤程度、炎症反应程度、Hcy代谢密切相关。
[Abstract]:Objective: to study the correlation between transcranial Doppler blood flow parameters and Hcy metabolism in patients with lacunar cerebral infarction. Methods: 88 patients with lacunar cerebral infarction (LI) were selected as Li group and 100 healthy volunteers as control group. The blood flow parameters of the middle cerebral artery and the basilar artery were measured by transcranial Doppler ultrasound. Results in the middle cerebral artery (MCA) and basilar artery (BAA) of the control group, the MFV Pi of the middle cerebral artery and the basilar artery were significantly higher than those of the control group, and the contents of MMP-9IL-18 and CD40LHcy in the serum of the control group were significantly higher than those in the control group, and were positively correlated with the MFVPI of the middle cerebral artery and the basilar artery, folic acid, and folic acid. The content of vitamin B12 was significantly lower than that of the control group and negatively correlated with the MFV Pi of the middle cerebral artery and basilar artery. Conclusion: in patients with lacunar cerebral infarction, the blood flow of middle cerebral artery and basilar artery is accelerated, pulsatility is enhanced, and the change of blood flow parameters of intracranial artery is closely related to the degree of nerve injury and the degree of inflammatory reaction and Hcy metabolism.
