高迁移率蛋白A2(HMGA2)在脑胶质瘤中的表达及其相关研究

【摘要】:研究背景 脑胶质瘤是最常见的中枢神经系统肿瘤之一,在中枢神经系统肿瘤中发病率仅次于脑膜瘤,位于第二位。随着现代医学对疾病认识水平的提高及CT、MR等先进检验技术的推广应用,胶质瘤的年发病率持续升高。胶质瘤具有高度增殖能力和侵袭性,是神经外科常见的致死性疾病之一。由于胶质瘤细胞的高度侵袭性,术中胶质瘤边界不清,手术全切难度大,临床预后不佳且复发率较高。 近年来,对于基于分子生物学的肿瘤诊断标志物及基因靶向治疗越来越受到重视。高迁移率蛋白A2(High mobility group protein A2,HMGA2)是一种调控基因转录的结构性转录因子,被发现在许多良恶性肿瘤的发生、增殖及转移过程中起重要作用,现被认为是潜在的诊断标志物和基因靶向治疗药物。 目的 探讨HMGA2和基质金属蛋白酶9(Matrix metalloproteinase9,MMP-9)在脑胶质瘤细胞中的表达及其与临床病理分型和侵袭性的关系。 材料与方法 收集了2012年2月至2012年10月在山东大学附属省立医院神经外科行手术切除保存完好的脑胶质细胞瘤标本57例,其中男性27例,女性30例。其中低度恶性(Ⅱ级)标本、中度恶性(Ⅲ级)、高度恶性(Ⅳ级)标本分别为23、18和16例。6例对照组标本取自脑外伤手术患者的正常脑组织。 采用免疫组化(S-P)方法检测HMGA2、MMP-9在不同级别脑胶质瘤标本中的表达情况。采用实时荧光定量聚合酶链反应(RT-PCR)方法测量HMGA2、MMP-9的mRNA在不同级别脑胶质瘤标本中的表达情况。并选择合适统计学方法对所得的结果进行分析,最终得出HMGA2、MMP-9之间的相关关系及其与肿瘤病理级别间的相关关系的结论。 结果 免疫组化结果显示,HMGA2在恶性胶质瘤细胞核表达率为91.2%(52/57),其中高表达(+++)29.8%(17/57)、中表达(++)47.4%(27/57)、低表达或阴性(+或0)22.8%(13/57),在正常脑组织细胞未见表达。且HMGA2在胶质母细胞瘤(P=0.033)与间变性星形细胞瘤(P=0.011)中的表达显著性高于星形细胞瘤,而在胶质母细胞瘤与间变性星形细胞瘤中的表达之间无显著性差异(P=0.923)。HMGA2表达与MMP-9(P<0.01)、Ki-67(P<0.01)显著性正相关。RT-PCR结果显示HMGA2在转录水平上与MMP-9(r=0.441,P=0.027)和Ki-67(r=0.498,P=0.011)表达显著性正相关。 结论 HMGA2的表达与代表肿瘤增值能力的Ki-67和细胞侵袭能力的MMP-9的表达呈显著性正相关关系。提示HMGA2的过表达可能增强了脑胶质瘤细胞的增殖及侵袭能力,可以作为脑胶质瘤的肿瘤诊断标志物和基因靶向治疗药物做进一步研究。

With the development of modern medicine and the application of advanced detection techniques such as CTN Mr, the annual incidence of glioma is increasing. Glioma has high proliferative ability and invasiveness and is one of the common fatal diseases in neurosurgery. Because of the high invasiveness of glioma cells, the boundary of glioma is not clear, the operation is difficult, the clinical prognosis is poor and the recurrence rate is high. In recent years, more and more attention has been paid to tumor diagnostic markers and gene targeting therapy based on molecular biology.
