血清超敏CRP及尿酸与2型糖尿病合并脑梗塞的临床研究

【摘要】:研究目的:研究血清hs-CRP、UA与2型糖尿病合并脑梗塞(DMCI)在颈动脉斑块稳定性、神经功能缺损程度、梗塞灶面积的相关性;研究血UA水平与血清hs-CRP的相关性;研究糖尿病合并脑梗塞患者头颅MRI特点。 研究方法:选择2012年7月至2013年4月在兰州大学第一医院神经内科住院的180例首次发病的急性脑梗塞患者,其中糖尿病合并脑梗塞组90例,非糖尿病脑梗塞组90例。所有患者行颈动脉彩超检查有无斑块及斑块性质,行头颅MRI检查明确梗塞灶的面积和部位,行NIHSS评分明确神经功能缺损程度,并测定hs-CRP、UA、同型半胱氨酸(Hcy)、血脂、血糖、糖化血红蛋白等含量。 研究结果:糖尿病合并脑梗塞组hs-CRP、UA含量高于非糖尿病脑梗塞组,差异有统计学意义;DMCI患者中,不稳定斑块组和稳定斑块组、神经功能中重度缺损组和轻度缺损组、大面积梗塞组和小梗塞组相比,hs-CRP、UA水平差异有统计学意义(P0.05)。 研究结论:hs-CRP、UA水平可反映DMCI组颈动脉斑块稳定性、神经功能缺损程度及脑梗塞面积大小。糖尿病合并脑梗塞主要特征是基底节区多发性腔隙性脑梗塞。脑梗塞急性期hs-CRP与UA水平呈正相关关系。
[Abstract]:Objective: to study the relationship between serum hs-CRPU UA and type 2 diabetes mellitus complicated with cerebral infarction (DMCI) in carotid plaque, nerve function defect, infarct area, serum UA level and serum hs-CRP. To study the characteristics of MRI in the head of diabetic patients with cerebral infarction. Methods: from July 2012 to April 2013, 180 patients with first onset acute cerebral infarction were selected from Department of Neurology, first Hospital of Lanzhou University, including 90 cases of diabetes complicated with cerebral infarction and 90 cases of non-diabetic cerebral infarction. All patients were examined by carotid artery color Doppler ultrasonography for plaque and plaque, head MRI was performed to determine the area and location of infarct, NIHSS score was used to determine the degree of neurological impairment, and hs-CRP UAA, homocysteine (Hcy), blood lipid, blood glucose were measured. Saccharified hemoglobin, etc. Results: the levels of hs-CRPU UA in diabetic patients with cerebral infarction were significantly higher than those in non-diabetic cerebral infarction patients. The difference was statistically significant among the unstable plaque group and stable plaque group, nerve function moderate and severe defect group and mild defect group. There was significant difference in UA level between large area infarction group and small infarction group (P0.05). Conclusion the results suggest that the level of% hs-CRPUA can reflect the stability of carotid plaque, the degree of nerve function defect and the size of cerebral infarction in DMCI group. The main characteristic of diabetes complicated with cerebral infarction is multiple lacunar infarction in basal ganglia. There was a positive correlation between hs-CRP and UA levels in acute cerebral infarction.
