脑膜瘤的MRI征象与MDM2和PTEN因子表达的临床应用研究

【摘要】：目的:研究不同级别脑膜瘤的影像特征与MDM2和PTEN表达的关系,探究不同级别脑膜瘤影像表现的分子病理基础,分析不同级别与MDM2,PTEN蛋白表达及关系,两种蛋白表达与MRI征象的关系,进而揭示肿瘤发生发展中生物学行为与影像征象的关联。方法:收集兰州大学第二医院2012年1月1日至2016年9月1日间经手术及病理证实的病例,术前均行MRI平扫及增强扫描,盲法记录各病例MRI特征,查找腊块进行MDM2、PTEN染色;所得数据应用Spss19.0统计软件进行统计学分析。结果:不同级别脑膜瘤MRI分叶征(χ2=32.018,P0.001)、瘤脑界面(χ2=20.292,P=0.001)、瘤周水肿(χ2=8.753,P=0.013)、坏死囊变(χ2=12.619,P=0.002)、强化均匀性(χ2=13.214,P=0.001)、邻近骨质改变(χ2=10.431,P=0.005)6项差异均有统计学意义(P均0.05),脑膜尾征(χ2=4.282,P=0.118)、肿瘤强化程度(χ2=5.603,P=0.061)及瘤内血管流空信号(χ2=3.617,P=0.164)3项差异无统计学意义(P均0.05)。脑膜瘤MDM2、PTEN蛋白免疫组化染色表达阳性,MDM2在脑膜瘤不同级别表达有差异(H=67.15,P0.001),呈正相关(r=0.889,P0.001);PTEN在脑膜瘤不同级别表达有差异(H=69.59,P0.001),呈负相关(r=-0.905,P0.001)。分叶征、瘤脑界面、瘤周水肿、坏死囊变、肿瘤强化后均匀性及邻近骨质改变6项MRI征象与MDM2表达呈正相关,6项MRI征象与PTEN表达呈负相关。结论:不同级别脑膜瘤的MRI特征和MDM2、PTEN表达存在差异,分叶征象、瘤脑界面模糊、瘤周水肿、坏死囊变、强化不均匀及邻近颅骨的改变等征象的鉴别意义较大,6项MRI征象与MDM2表达呈正相关,与PTEN表达呈负相关,揭示了不同级别脑膜瘤MRI征象与MDM2及PTEN表达相关性,为提高脑膜瘤分级诊断、临床治疗和预后判断提供依据。
[Abstract]:Objective: to study the relationship between the imaging features of meningiomas of different grades and the expression of MDM2 and PTEN, to explore the molecular pathological basis of the imaging manifestations of meningiomas of different grades, and to analyze the expression and relationship between MDM2,PTEN protein and different grades of meningiomas. The relationship between the expression of two proteins and MRI signs, and the relationship between biological behavior and imaging signs in tumorigenesis and development were revealed. Methods: cases confirmed by operation and pathology from January 1, 2012 to September 1, 2016 in the second Hospital of Lanzhou University were collected.
