交感皮肤反应、肛门括约肌肌电图检查在PD自主神经功能障碍判断中的作用

【摘要】:目的探讨交感皮肤反应(SSR)、肛门括约肌肌电图(EAS-EMG)判断帕金森病患者自主神经功能障碍的价值。方法选取50例帕金森病患者(帕金森病组),42例健康体检者作为对照组;两组均行帕金森病自主神经症状量表(SCOPA-AUT)评分判断自主神经功能,行SSR和EAS-EMG检查。分析SCOPA-AUT评分与SSR、EASEMG的相关性。结果帕金森病组与对照组SCOPA-AUT评分分别为(32.30±14.38)、(3.29±2.07)分,两组比较P0.01。帕金森病组自主神经功能障碍(PD-AS)39例(78%),不伴自主神经功能障碍(PD-NAS)11例(22%)。与对照组比较,帕金森病组PD-AS者SSR潜伏期延长、波幅降低(P均0.05);PD-AS者与PD-NAS者SSR异常率分别为82.1%、18.2%,二者比较P0.01。EAS-EMG检测结果显示,帕金森病组PD-AS者运动单位动作电位(MUAP)平均时限、多相波百分比显著高于PD-NAS者(P均0.05)。帕金森病组SCOPA-AUT评分与上、下肢SSR潜伏期均呈正相关(r分别为0.492、0.642,P均0.05);与EAS-EMG的MUAP平均时限、多相波百分比均呈正相关(r分别为0.473、0.516,P均0.05)。结论 SSR和EAS-EMG可用于评价帕金森病患者自主神经功能障碍严重程度。
[Abstract]:Objective to investigate the value of (SSR), anal sphincter electromyography (EAS-EMG) in the diagnosis of autonomic nervous dysfunction in patients with Parkinson's disease. Methods 50 patients with Parkinson's disease (Parkinson's disease group) and 42 healthy controls were selected as control group. The autonomic nervous function was assessed by SCOPA-AUT, SSR and EAS-EMG were performed in both groups. The correlation between SCOPA-AUT score and SSR,EASEMG was analyzed. Results the SCOPA-AUT score of Parkinson's disease group and control group was (32.30 卤14.38), (3.29 卤2.07), compared with that of control group (P 0.01). In Parkinson's disease group, there were 39 cases (78%) with autonomic nervous dysfunction (PD-AS) and 11 cases (22%) with no autonomic nerve dysfunction (PD-NAS). Compared with the control group, the SSR latency and amplitude of SSR were prolonged and the amplitude was decreased in PD-AS patients with Parkinson's disease, the abnormal rate of SSR in PD-AS and PD-NAS patients was 82.1 and 18.2, respectively. The results of P0.01.EAS-EMG test showed that the mean duration of (MUAP) of motor unit action potential in PD-AS patients with Parkinson's disease was higher than that in control group (P < 0.05). The percentage of multiphase wave was significantly higher than that of PD-NAS (all P 0. 05). The SCOPA-AUT score was positively correlated with the upper and lower limb SSR latency (r = 0.492 卤0.642a, P 0.05, respectively), and positively correlated with the average MUAP duration and the percentage of multiphase waves of EAS-EMG (r = 0.473 卤0.516, P 0.05, respectively). Conclusion SSR and EAS-EMG can be used to evaluate the severity of autonomic nervous dysfunction in patients with Parkinson's disease.
