非溶栓性缺血性卒中患者出血性转化的危险因素分析

【摘要】:目的探讨与非溶栓性缺血性卒中患者出血性转化相关的危险因素。方法回顾性分析2014年1月-2016年12月我院收治的非溶栓性缺血性卒中患者共计426例,收集其基本信息、临床指标和影像学的资料,采用多元Logistic回归模型分析非溶栓性缺血性卒中出血性转化相关的危险因素。结果在我们纳入的病例中出血性转化(hemorrhagic transformation,HT)的发生率为17.3%。多元Logistic回归分析表明,高血压病(OR 0.463,95%CI 0.276~0.774)、房颤(OR 3.814,95%CI 1.870~7.775)、大面积脑梗死(OR 2.883,95%CI 1.506~5.520)及肾小球滤过率(estimated glomerular filtration rate,eGFR)60mL/min/1.73 m2(OR1.993,95%CI 0.994~3.995)是发生出血性转化的独立危险因素(P0.05)。结论高血压病、房颤、大面积脑梗死及eGFR60mL/min/1.73 m2是非溶栓性缺血性卒中出血性转化的独立危险因素;与既往研究相比,我们的研究资料提示检测eGFR指标对预测非溶栓性缺血性卒中出血性转化有一定潜在价值。
[Abstract]:Objective to investigate the risk factors associated with hemorrhagic transformation in patients with non-thrombolytic ischemic stroke. Methods 426 patients with non-thrombolytic ischemic stroke from January 2014 to December 2016 were retrospectively analyzed and their basic information, clinical indexes and imaging data were collected. Multivariate Logistic regression model was used to analyze the risk factors associated with hemorrhagic transformation in non-thrombolytic ischemic stroke. Results the incidence of hemorrhagic transformation (hemorrhagic transformation,HT) in our patients was 17. 3%. Multivariate Logistic regression analysis showed that hypertension (OR 0.46395 CI 0.276c0.774), atrial fibrillation (OR 3.81495 CI 1.8707.775), massive cerebral infarction (OR 2.88395 CI 1.5065.520) and glomerular filtration rate (estimated glomerular filtration rate,) EGFR) 60mL/min/1.73 m2 (OR1.993,95%CI 0.994 + 3.995) was an independent risk factor for hemorrhagic transformation (P0.05). Conclusion Hypertension, atrial fibrillation, large area cerebral infarction and eGFR60mL/min/1.73 m2 are independent risk factors for hemorrhagic transformation of non-thrombolytic ischemic stroke. Compared with previous studies, our data suggest that the detection of eGFR is of potential value in the prediction of hemorrhagic transformation in non-thrombolytic ischemic stroke.
