颅内皮样囊肿的诊治进展

【摘要】：颅内皮样囊肿是一种少见的先天性良性肿瘤,其临床表现取决于病灶的位置、大小、生长速度、有无破裂与感染等,无特征性症状/体征。典型的皮样囊肿在CT上表现为边界清楚的低密度影,CT值多为负值,周围水肿缺如,在MRI上,T1、T2加权以高信号为主,囊肿多不被增强,而囊壁可有条索状强化。确诊需行病理学检查。治疗首选开颅显微镜下手术切除。大多数患者预后良好,但也可复发、恶变、并发无菌性脑膜炎和脑积水,所有患者都应长期随访。
[Abstract]:Intracranial dermoid cyst is a rare congenital benign tumor. Its clinical manifestation depends on the location, size, growth rate, rupture and infection of the lesion, and has no characteristic symptoms / signs. On CT, typical dermoid cysts showed low density with clear boundary, negative CT value and absent peripheral edema. On MRI, T _ 1 T _ 2 weighted images were mainly hyperintense, most cysts were not enhanced, and the cyst wall could be enhanced in stripe shape. Pathological examination is necessary for diagnosis. The first choice of treatment is craniotomy under the microscope. Most patients have a good prognosis, but can also relapse, malignant change, accompanied by aseptic meningitis and hydrocephalus, all patients should be followed up for a long time.
