血糖水平对急性脑梗塞早期预后的影响

【摘要】:目的主要研究血糖水平对急性脑梗塞早期预后的影响。 方法采用前瞻性设计,连续收录我院104例发病72h内入院急性脑梗塞病人,检测空腹静脉血糖,病人评估采用美国国立卫生研究院卒中量表(NIHSS),运用改良Rankin残障量表评分(mRs)在发病30天时评估预后。统计学采用Logistic回归分析,分析影响血糖水平的因素及高血糖对急性脑梗塞早期预后的影响,P0.05有统计学差异。 结果(1)104例急性脑梗塞患者中,43例(41.3%)出现高血糖,年龄、糖尿病史、NIHSS评分是急性脑梗塞合并高血糖的危险因素(P0.05),糖尿病史、NIHSS评分是急性脑梗塞合并高血糖的独立危险因素(P0.05)。(2)感染、高血糖、NIHSS评分对mRs评分的影响显著相关(P0.05),高血糖.NIHSS评分是急性脑梗塞早期预后不良的独立危险因素(P0.05)。 结论高血糖为急性脑梗塞早期预后差的独立危险因素。
[Abstract]:Objective to study the effect of blood glucose level on early prognosis of acute cerebral infarction. Methods A prospective design was used to measure fasting venous blood glucose in 104 patients with acute cerebral infarction admitted to our hospital within 72 hours of onset. The patients were evaluated with the National Institutes of Health Stroke scale (NIHSS),). The modified Rankin Disability scale (mRs) was used to evaluate the prognosis 30 days after onset. Logistic regression analysis was used to analyze the factors affecting blood glucose level and the influence of hyperglycemia on early prognosis of acute cerebral infarction (P0.05). Results (1) among 104 patients with acute cerebral infarction, 43 (41.3%) had hyperglycemia, age, history of diabetes, NIHSS score were risk factors of acute cerebral infarction with hyperglycemia (P0.05), history of diabetes mellitus, NIHSS score was an independent risk factor of acute cerebral infarction complicated with hyperglycemia (P0.05). (2) infection. The influence of hyperglycemia and NIHSS score on mRs score was significant (P0.05). Hyperglycemia and NIHSS score were independent risk factors for poor early prognosis of acute cerebral infarction (P0.05). Conclusion hyperglycemia is an independent risk factor for poor early prognosis of acute cerebral infarction.
