较低温度局麻下射频热凝术治疗三叉神经痛的有效性

【摘要】：目的观察局麻下较低的不同温度射频热凝术(RFT)治疗三叉神经痛(TN)的有效性、复发率、并发症及患者生活质量情况。方法选取2013年1月至2015年12月就诊于中国医科大学附属盛京医院疼痛科的原发性TN患者600例,在CT引导、局麻下行RFT治疗,温度分别为65℃(A组)、70℃(B组)、75℃(C组)。观察术中并发症及术后3年内疼痛缓解、复发率、并发症及患者生活质量情况。结果手术均顺利完成。B组与C组的疼痛缓解率及复发率无差异,且优于A组。并发症方面,A组最少,B组次之,C组并发症最多,恢复时间最长。B组患者生活质量满意度最高。结论 65℃和75℃RFT均可以有效缓解TN,并发症随着温度的升高而增加。但是70℃与75℃RFT的疼痛缓解率及复发率一致,并发症70℃RFT少于75℃RFT,因此建议选择70℃以下温度RFT治疗原发性TN。
