三维颅脑容积成像增强扫描序列对脑膜癌病累及颅神经的诊断价值

目的通过对比MR三维颅脑容积成像(3D-BRAVO)增强扫描序列与常规增强扫描序列在诊断脑膜癌病累及颅神经中的区别,探讨3D-BRAVO增强序列在脑膜癌病累及颅神经诊断中的价值及脑膜癌病中受累颅神经的分布特点。方法回顾性分析2015年01月至2020年03月于中国联勤保障部队第904医院进行诊治的具有完整影像资料及临床资料的脑膜癌病患者114例。所有患者均行头颅常规MR平扫及增强扫描,增强扫描序列包括常规增强扫描序列及3D-BRAVO增强扫描序列,并进行图像后处理。分析脑膜癌病在两种增强扫描中的特征以及对比各组受累颅神经的检出率。结果在114例脑膜癌病患者中,脑膜异常强化97例,其中硬脑膜-蛛网膜强化12例,软脑膜-蛛网膜下腔强化74例,混合型强化11例。影像学中颅神经强化患者有81例,发生率为71.05%,采用卡方检验分析,在脑膜癌病累及颅神经的诊断中,3D-BRAVO增强扫描序列与常规MR增强扫描序列二者有统计学差异,3D-BRAVO增强扫描序列优于常规增强扫描序列;本研究显示受累神经中面听神经的发生率为58.77%,三叉神经发生率为47.37%、动眼神经发生率为9.65%、舌下神...
