成人近视性屈光参差LASIK术后融合范围与立体视的变化

【摘要】:目的:观察准分子激光原位角膜磨镶术(laser in situ keratomileusis, LASIK)矫治成人近视性屈参差术前及术后融合范围和立体视变化,从视觉功能的角度上去评价LASIK手术的疗效,为LASIK术后视觉质量预测提供一定临床依据。 方法:选取的对象是2013-1/2013-4期间在我院进行LASIK手术治疗的近视成人患者60例(120眼),按双眼等效球镜差≥2.0D分为屈光参差组与非屈光参差组各30例(60眼),随诊6个月,分别于术前,术后10天,术后1个月,术后3个月,术后6个月检查视力、融合范围、远和近立体视,比较两组差异。 结果:60例患者经LASIK术后裸眼视力均达到0.8以上,屈光参差组的双眼屈光参差程度明显减小。 立体视:非屈光参差组术后1个月较术前显著改善(χ2=10.976,P=0.027),,其后渐趋平稳;屈光参差组远、近立体视均有较明显改善,近立体视术后1个月较术前比较有统计学意义(χ2=8.298,P=0.016),远立体视术后1个月较术前差异有统计学意义(χ2=7.084,P=0.029)。 融合范围:随时间推移先减低后升高(P=0.003),两组术后3个月比较术前改善明显(P0.05),另外,术后3个月时屈光参差组比较非屈光参差组差异有统计学意义(t=2.201,P=0.032)。 结论:由各种原因导致的近视将致使双眼视觉功能有着不同程度的损害,如立体视觉和融合范围。LASIK手术后双眼恢复正视,立体视觉和融合范围逐渐恢复正常范围。对于屈光参差组,LASIK术消除双眼视像不等,使得原本模糊的视网膜成像变得清晰,促进双眼单视功能的恢复,从而立体视觉和融合范围得到改善。
[Abstract]:Objective: to observe the range of fusion and stereopsis before and after laser in situ keratomileusis (LASIK) for correction of myopia anisometropia in adults, and to evaluate the effect of LASIK operation from the angle of visual function. To provide a certain clinical basis for the prediction of visual quality after LASIK. Methods: sixty adult myopic patients who underwent LASIK operation in our hospital during 2013-1 / 2013-4 were divided into anisometropia group (n = 30) and non-anisometropia group (n = 30). Visual acuity, fusion range, distant and proximal stereopsis were examined 10 days, 1 month, 3 months and 6 months after operation. Results the naked visual acuity of 60 patients with LASIK was better than 0. 8, and the degree of anisometropia was decreased in the anisometropia group. Stereopsis: in the anisometropia group, there was a significant improvement at 1 month after operation (蠂 ~ 2 / 10.976 / P 0.027) and then became more stable, while in the anisometropia group, there was a significant improvement in the distance and near stereopsis, there was a significant improvement in the anisometropia group (P < 0.05). One month after operation, there was a significant difference between the two groups (蠂 ~ (2) 8.298P ~ (0.016) and one month after distant stereopsis (蠂 ~ (2) 7.084) (蠂 ~ (2) 7.084 (P ~ (0.029). The range of fusion: decrease first and then increase P0. 003 with the passage of time. The improvement of P0. 05 was significant in the two groups 3 months after operation. In addition, there was a significant difference between the anisometropia group and the non anisometropia group at 3 months after operation. Conclusion: myopia caused by various causes will result in different degrees of damage to binocular visual function, such as stereoscopic vision and fusion range. For anisometropia group, LASIK eliminates the disparity of binocular vision, makes the original blurred retinal imaging clear, promotes the recovery of binocular monocular function, and improves the range of stereo vision and fusion.
