海藻酸盐纤维医用止血敷料的评价研究

【摘要】:目的研究以海藻酸盐为代表的海洋生物提取物制备而成的医用敷料的性能,分别从止血效果、生物相容性、抗菌性三方面内容进行分析,并与目前临床上常用的普通纱布、明胶止血海绵等止血敷料作对照,探讨以海藻酸盐为原料制备的医用敷料的临床应用价值。方法试验一,利用体外止血试验建立动物出血模型,将以上三种止血材料敷于新西兰兔背部两侧创口。记录出血时间,收集止血敷料计算止血前后质量差值,以及用氰化高铁血红蛋白检测试剂测止血后敷料洗涤下的血红蛋白吸光度值,从以上三方面评价止血效果。试验二,先制备各受试敷料浸提液,再分别进行体外溶血试验、急性全身毒性试验以及CCK-8体外细胞毒性试验。试验三,采用振荡法评价敷料间对金黄色葡萄球菌的抗菌性差异。结果试验一,海藻酸盐敷料、纱布、明胶止血海绵出血时间依次为95.28±3.95s、138.33±5.98s、101.25±4.53s(F=2433.23,P0.05),Hb光度吸收值依次为1.269±0.011、1.146±0.013、1.754±0.013(F=24764.804,P0.05),出血量依次为0.921±0.021g、0.900±0.012g、1.607±0.028g(F=3364.499,P0.05)。试验二,海藻酸盐敷料浸提液的溶血率为0.8%;注射海藻酸盐敷料浸提液的小鼠,症状较阴性对照组小鼠无明显差异,没有发生急性全身毒性反应;海藻酸盐敷料的细胞毒性评价为1级;试验三,海藻酸盐敷料的抗菌率为49%。结论海藻酸盐纤维敷料止血速度快、能有效控制出血对象的出血量,止血效果好,综合评价止血效果优于明胶海绵与纱布;海藻酸盐纤维敷料细胞毒性小、急性全身毒性反应低、溶血率低,均符合国家规定的使用标准,表现出有良好的生物相容性;海藻酸盐纤维敷料对金黄色葡萄球菌的增殖有抑制作用且明显优于普通纱布,显示良好的抗菌性。
[Abstract]:Objective to study the properties of medical dressings prepared from marine extracts represented by alginate, and to analyze the effects of hemostasis, biocompatibility and antimicrobial activity, and to compare them with common gauze, which are commonly used in clinical practice. Compared with gelatin hemostatic sponge and other hemostatic dressings, the clinical application value of medical dressings made from alginate was discussed. Methods in experiment 1, the animal bleeding model was established by in vitro hemostasis test, and the above three kinds of hemostatic materials were applied to the bilateral wounds in the back of New Zealand rabbits. The bleeding time was recorded, the quality difference before and after hemostasis was calculated by collecting the hemostatic dressing, and the hemoglobin absorbance value of the dressing washed after hemostasis was measured with cyanomethemoglobin test reagent, and the hemostatic effect was evaluated from the above three aspects. In the second experiment, the extracts of each dressing were prepared, and then hemolysis test, acute systemic toxicity test and CCK-8 cytotoxicity test in vitro were carried out respectively. In experiment 3, the difference of antibacterial activity between dressings against Staphylococcus aureus was evaluated by oscillating method. Results in experiment 1, the photometric absorption time of alginate dressing, gauze and gelatin hemostatic sponge was 95.28 卤3.95s-1 138.33 卤5.98sm 101.25 卤4.53sm FU 2433.23P0.05Hb, respectively: 1.269 卤0.0111.146 卤0.0131.754 卤0.013174.804, and the bleeding volume was 0.921 卤0.021g / 0.900 卤0.012g1.607 卤0.028gF1. 607 卤0.028gF1 / 3364.499P0.05A, respectively. The results showed that the photometric absorption time of alginate dressing, gauze and gelatin hemostatic sponge was 1.269 卤0.0111.146 卤0.0131.754 卤0.013764.804 and 0.921 卤0.021g / 0.900 卤0.012g1.607 卤0.028g / L respectively. In experiment 2, the hemolysis rate of the extract of alginate dressing was 0.8.The mice injected with alginate dressing had no significant difference in symptoms and had no acute systemic toxic reaction compared with the negative control group. The cytotoxicity evaluation of alginate dressing was grade 1, and the antibacterial rate of alginate dressing was 49%. Conclusion the alginate fiber dressing has the advantages of fast hemostatic speed, effective control of bleeding volume and good hemostatic effect, and the comprehensive evaluation of hemostatic effect is superior to that of gelatin sponge and gauze, and the cytotoxicity of alginate fiber dressing is less than that of gelatin sponge and gauze. The acute systemic toxicity was low, the hemolysis rate was low, all of them were in accordance with the national standard of use and showed good biocompatibility, the alginate fiber dressing had inhibitory effect on the proliferation of Staphylococcus aureus and was obviously superior to that of the common gauze. Show good antibacterial properties.
