文章摘要：目的通过研究发现影响泌尿外科老年患者下肢活动依从性的因素，制订切实可行的下肢活动方案，预防患者术后下肢静脉血栓的形成。方法在泌尿外科病房选取研究者所在责任组内行手术治疗患者共22例进行下肢活动指导，，根据Lewin提出的行动研究框架，以计划、行动、观察、反思4个步骤进行螺旋式循环，评价活动方案的有效性和可行性，采用询问法评估患者的依从性。结果实践10周，共完成4轮循环。经过实践细化护理程序，完善危险因素评估表，调整宣教时间等，使被干预患者对下肢活动重要性的认知程度和接受程度得到了提高，活动的依从性良好。结论本研究所制订的下肢活动方案经过行动研究实践的修改及完善。能够提高患者下肢活动的依从性，以预防深静脉血栓的发生。

Abstr：Objective To explore the influential factors of compliance of lower limbs exercise,and to develop efficient program of lower limbs exercise to prevent deep venous thrombosis（DVT） for elderly patients after urinary surgery. Methods Twenty-two elderly patients after urinary surgery were recruited in this study. The patients were asked to do lower limbs exercises guided by the nurses. Based on the framework of Lewin＇s action research, researchers evaluated the effectiveness and feasibility of the lower limbs exercise program with the cycles of diagnosing, planning, action taking,observing and reconsidering. In addition, the compliance of patients was assessed through interview. Results After ten weeks of practice, 4 cycles has been finished. In this period,the researchers improved the evaluation form of dangerous factors,refined the nursing process and adjusted the education schedule, which improved the patients＇ recognition and acceptance to the importance of lower limbs exercise, and promoted the compliance of lower limbs exercise. Conclusion The lower limbs exercise program has been revised and perfected through action research and has been proved to be effective to prevent the incidence of DVT.
