术前放疗加手术与单纯手术治疗喉癌生存率比较的Meta分析

【摘要】：[目的]比较术前放疗加手术与单纯手术治疗喉癌的疗效,为喉癌治疗方式的选择提供可靠的循证依据。[方法]检索国内外有关数据库查找符合检索条件的前瞻性临床随机对照试验,采用Meta分析的方法,对国内外公开发表的有关术前放疗加手术与单纯手术治疗喉癌的临床随机对照试验的研究文献进行综合分析。[结果]共纳入5个随机对照试验,研究对象共1086例。术前放疗加手术与单纯手术相比并未提高喉癌患者的3和5年生存率,RR分别为0.94(95%CI为0.71~1.24)和1.09(95%CI为0.88~1.34)。术前放疗加手术与单纯手术相比治疗声门上型、声门型以及Ⅱ、Ⅲ、Ⅳ期喉癌3年生存率,差异均无统计学意义,RR分别为0.93(95%CI为0.61~1.42,P=0.73)、0.55(95%CI为0.26~1.19,P=0.13)、0.86(95%CI为0.34~2.15,P=0.74)、0.82(95%CI为0.46~1.47,P=0.50)和0.74(95%CI为0.44~1.25,P=0.26)。术前放疗加手术与单纯手术相比治疗声门上型、声门型以及Ⅰ/Ⅱ和Ⅲ/Ⅳ期喉癌5年生存率,差异均无统计学意义,RR分别为1.01(95%CI为0.80~1.28,P=0.92)、1.06(95%CI为0.67~1.67,P=0.80)、0.97(95%CI为0.86~1.09,P=0.63)和1.01(95%CI为0.90~1.13,P=0.91)。[结论 ]本系统评价纳入研究有限,结果提示术前放疗加手术治疗对比手术治疗并未提高喉癌患者的生存率,需进一步开展更多高质量临床研究进行验证。
[Abstract]:[Objective] to compare the curative effect of preoperative radiotherapy plus operation on laryngeal carcinoma and to provide reliable evidence-based basis for the choice of treatment mode of laryngeal carcinoma. [Methods] A prospective randomized controlled trial was used to search the relevant databases at home and abroad, which met the search criteria. The method of Meta analysis was used. The literature published at home and abroad on clinical randomized controlled trials of preoperative radiotherapy plus surgery and simple surgery for laryngeal carcinoma was analyzed. [Results] A total of 1 086 patients were enrolled in 5 randomized controlled trials. Preoperative radiotherapy plus surgery did not improve the 3 and 5 year survival rate of laryngeal cancer patients compared with surgery alone.
