【摘要】：目的探讨耳后超薄皮瓣在耳廓再造术中的安全性、术后效果。方法选取我院2014年5月~2016年5月收治的先天性小耳畸形患者20例作为研究对象。整形外科采用耳后超薄皮瓣包裹自体肋软骨支架对20例、21耳先天性小耳畸形患者行耳郭再造术。结果 20例共21耳完成耳廓再造,18例(85%)顺利完成三期,未出现相关并发症。结论 (1)利用耳后超薄皮瓣包裹自体肋软骨支架行耳廓再造,取代筋膜瓣覆盖加游离植皮,再造耳形态立体、自然,各解剖亚单位结构清晰,耳颅角形态自然;(2)耳后超薄皮瓣下方保留三角瓣,增加耳后面积,减少耳后瘢痕增生;(3)减少扩张法注水过程,减少患者痛苦;(4)配合激光去除皮瓣远端残留毛发,效果更佳;(5)保留耳后筋膜及颞浅筋膜,减少手术创伤,该结构在软骨支架外露,皮肤缺损时行修复使用。
[Abstract]:Objective to investigate the safety of retroauricular skin flap in auricle reconstruction. Methods from May 2014 to May 2016, 20 patients with congenital microauricular malformation were selected as the study objects. Plastic surgery was performed by using a super-thin posterior ear flap wrapped with autogenous costal cartilage scaffold. 20 cases. 21 ears with congenital microauricular malformation underwent auricle reconstruction. Results 20 cases (21 ears) completed auricle reconstruction in 18 cases (85 cases). Conclusion 1) Autologous costal cartilage scaffold was reconstructed by using super-thin posterior ear flap, instead of fascia flap and free skin graft, the shape of ear reconstructed was stereoscopic and natural.Conclusion\\\; The structure of each anatomical subunit was clear and the cranial horn was naturally formed. (2) the triangular flap was retained below the ultra-thin posterior ear flap to increase the area behind the ear and reduce the scar hyperplasia; (3) reducing the process of water injection by dilatation and reducing the pain of the patients; (4) combined with laser to remove the residual hair from the distal end of the flap, the effect is better; (5) preserving the posterior ear fascia and superficial temporal fascia to reduce the surgical trauma. The structure is used when the cartilage scaffold is exposed and the skin defect is repaired.
