CXCL13及其受体CXCR5在喉鳞状细胞癌中的表达及其临床意义

【摘要】:目的:观察趋化因子CXCL13及其受体CXCR5在喉鳞状细胞癌组织及癌旁组织中的表达情况,并分析其与临床病理参数(包括性别、年龄、原发部位、分化程度、TNM分期、淋巴结转移与否、临床分期)及预后之间的相关性,从而探讨其临床意义。材料与方法:收集2000年2月——2015年6月期间于山西医科大学第一附属医院行喉鳞状细胞癌手术患者的手术切除组织包埋的石蜡标本。选取132例喉鳞癌石蜡组织标本及对应66例癌旁正常黏膜石蜡组织标本(距肿瘤边缘5mm的癌旁正常黏膜组织)。选取标本病理资料完整,经病理诊断均为喉鳞状细胞癌,且均为原发,术前均未接受放疗及化疗。采用免疫组织化学法检测喉鳞状细胞癌和癌旁正常黏膜石蜡包埋组织中CXCL13及CXCR5蛋白的表达情况。用数字病理切片扫描仪扫描经免疫组化方法处理好的切片,用CaseViewer1.4图片采集软件阅片审核,并判定棕色及黄褐色区域为阳性表达。采用SPSS Version20.0统计软件对实验结果进行统计学分析。包括χ2检验,Spearman等级相关分析及Kaplan-Meier生存分析及COX回归分析,检验水准α=0.05,P≤0.05时差异具有统计学意义。结果:1、CXCL13及CXCR5在喉鳞癌组织和正常组织中的表达:CXCL13在喉鳞癌组织中阳性率为50.8%,癌旁组织中阳性率为3.0%;CXCR5在喉鳞癌组织中阳性率为45.5%,在癌旁组织中阳性率为4.5%,CXCL13及CXCR5在癌和癌旁正常组织中的表达情况相比存在差异,具有统计学意义(P0.05)。2、CXCL13及CXCR5在喉鳞癌组织中的表达情况与临床病理参数的相关性:CXCL13及CXCR5蛋白表达强度与性别、年龄无明显相关性(P0.05),而与TNM分期、淋巴结转移情况及临床分期呈显著相关(P0.05);CXCL13及CXCR5的表达强度在声门型喉癌组织与声门上型喉癌组织中具有显著差异(P0.05);CXCL13的表达与分化程度相关(P=0.05)。3、CXCL13与CXCR5在喉鳞癌组织中表达情况的相关性及与患者生存时间的相关性:在喉鳞癌组织中,CXCL13与CXCR5的表达具有显著相关性;CXCL13及CXCR5低表达患者与高表达患者的生存时间相比差异具有统计学意义(P0.05)。结论:CXCL13及CXCR5在喉鳞癌组织中的阳性表达率显著高于癌旁组织;分析临床病理参数与CXCL13及CXCR5表达水平之间的相关性发现:CXCL13及其受体CXCR5蛋白的表达水平可以作为评估喉鳞癌发病及可能发生侵袭、转移的新指标;通过比较不同表达水平的喉鳞状细胞癌患者的生存时间,提示CXCL13及CXCR5可能为喉癌患者的不良预后因子。
[Abstract]:Objective: to observe the expression of chemokine CXCL13 and its receptor CXCR5 in laryngeal squamous cell carcinoma (LSCC) and its adjacent tissues, and to analyze its relationship with clinicopathologic parameters (including sex, age, primary location). The correlation between TNM stage, lymph node metastasis, clinical stage and prognosis. Thus, the clinical significance, materials and methods are discussed. Paraffin wax specimens were collected from patients with laryngeal squamous cell carcinoma (LSCC) surgically performed from February 2000 to June 2015 in the first affiliated Hospital of Shanxi Medical University. 132 cases of laryngeal squamous cell carcinoma (LSCC) were selected as paraffin wax. Tissue specimens and paraffin tissues corresponding to 66 normal mucosa adjacent to cancer (. The normal mucosa adjacent to the tumor was 5 mm from the margin of the tumor. The pathological data of the specimens were complete. All patients were diagnosed as laryngeal squamous cell carcinoma by pathology, and all were primary. The expression of CXCL13 and CXCR5 in paraffin embedded tissues of laryngeal squamous cell carcinoma and adjacent normal mucosa was detected by immunohistochemical method. The expression of CXCL13 and CXCR5 protein in paraffin embedded tissues of laryngeal squamous cell carcinoma and adjacent normal mucosa were detected by digital pathological section. Scanners scan sections processed by immunohistochemistry. The CaseViewer1.4 image acquisition software was used to read the film.
