多重聚合酶链反应法在急性感染性结膜炎诊断中的临床意义

【摘要】:目的:探讨多重聚合酶链反应技术(multiplex PCR,MPCR)对急性感染性结膜炎病因诊断中的应用价值。 方法:通过建立标准细菌株和致病性腺病毒株的MPCR体系,对根据临床表现拟诊为典型急性细菌性结膜炎组38眼、典型急性病毒性结膜炎组35眼、非典型急性结膜炎组30眼、正常对照组28眼等共131眼(131例)的结膜囊分泌物进行MPCR检测及细菌培养,并对其结果进行对比分析。 结果:(1)在本次MPCR体系中标准表皮葡萄球菌、金黄色葡萄球菌、大肠杆菌、肺炎链球菌DNA均扩增出520bp大小特异性条带。(2)典型急性细菌性结膜炎组(Ⅰ组)中扩增出520bp条带的有25眼(阳性率65.8%),明显高于典型急性病毒性结膜炎组(Ⅱ组)7眼(阳性率20.0%)(χ2=15.516,P0.001);Ⅱ组1004bp条带的有25眼(阳性率60.0%)明显高于Ⅰ组的6眼(阳性率17.1%)(χ2=17.071,P0.001)。(3)Ⅰ组和非典型急性结膜炎组(Ⅲ组)共68眼中扩增出520bp条带的有40眼(阳性率58.8%),细菌培养阳性率为67.6%,两种检测方法,差异无统计学意义(χ2=1.139,P=0.286)。(4)Ⅰ组、Ⅱ组、Ⅲ组与Ⅳ组(正常对照组)各比较MPCR阳性率后,结果均有显著统计学差异(P0.001),Ⅰ组、Ⅱ组、Ⅲ组的MPCR阳性检测率明显高于Ⅳ组。(5)Ⅰ组、Ⅲ组、Ⅳ组的MPCR细菌检出率与细菌培养阳性率比较结果,只有Ⅳ组的两种检测方法之间存在统计学差异(χ2=10.308,P=0.036)正常对照组的细菌培养检测率高于MPCR检测率。 结论:(1)急性细菌和病毒性结膜炎的MPCR检测率具有显著性差异,说明MPCR检测具有良好的特异性。(2)MPCR检测法对急性感染性结膜炎进行病因诊断具有临床应用意义。此方法与细菌培养法相比有速度快、特异性强、灵敏度高、经济简便等优势。
[Abstract]:Objective: to evaluate the value of multiplex PCR technique in the etiological diagnosis of acute infectious conjunctivitis. Methods: by establishing MPCR system of standard bacterial strain and pathogenic adenovirus strain, 38 eyes of typical acute bacterial conjunctivitis group and 35 eyes of typical acute viral conjunctivitis group were diagnosed according to clinical manifestations. The secretion of conjunctival sac in 30 eyes of atypical acute conjunctivitis group and 28 eyes of normal control group (131 eyes) were detected by MPCR and bacterial culture, and the results were compared and analyzed. Results in this MPCR system, Staphylococcus epidermidis, Staphylococcus aureus, Escherichia coli were standard. 520bp size-specific bands were amplified from Streptococcus pneumoniae DNA. 520bp bands were amplified from typical acute bacterial conjunctivitis group (group 鈪,
