梅毒性脉络膜视网膜炎患者眼底光学相干断层扫描特征

【摘要】：目的探讨梅毒性脉络膜视网膜炎患者眼底光学相干断层扫描(optical coherence tomography,OCT)特征。方法采用回顾性研究,纳入梅毒性脉络膜视网膜炎患者58例88眼,进行眼底荧光血管造影(fluorescence fundus angiography,FFA)、眼底吲哚青绿造影(indocyanine green angiography,ICGA)和OCT检查,以及快速血浆反应素环状卡片试验(rapid plasma reagin test,RPR)和梅毒螺旋体颗粒凝集试验(treponema pallidum particle agglutination test,TPPA),后给予驱梅治疗并随访。结果本研究中87眼视网膜色素上皮层针尖样、锯齿样突起,86眼视网膜外界膜及肌样体带、椭圆体带结构不清、消失,68眼玻璃体内高反射点,16眼视网膜浅脱离。驱梅治疗后,87眼中79眼视网膜色素上皮层针尖样、锯齿样突起完全恢复,86眼中62眼视网膜外界膜及肌样体带、椭圆体带部分显示,16眼视网膜浅脱离均恢复。结论梅毒性脉络膜视网膜炎眼底OCT主要特征为受累视网膜色素上皮针尖样、锯齿样突起,外界膜及肌样体带、椭圆体带显示不清或消失以及视网膜浅脱离,早期正规治疗后上述特征恢复良好。
[Abstract]:Objective to investigate the fundus optical coherence tomography (coherence) features in patients with syphilis chorioretinitis. Methods 88 eyes of 58 patients with chorioretinitis were studied retrospectively. Fundus fluorescein angiography (FFA) and fundus indocyanine green angiography (OCT) were performed. Rapid plasma reagin test (RPRR) and Treponema pallidum particle agglutination test (TPPAA) were followed up and followed up in 87 eyes. Of 86 eyes with serrated protuberance and myoid zone, the structure of the ellipsoid zone was unclear, and 68 eyes of the retinal superficial detachment with high intravitreous reflex point disappeared. In 62 eyes of 86 eyes, the outer membrane and myoid zone of the retina were completely recovered from the serrated protrusions. Conclusion the fundus OCT of 16 eyes with chorioretinitis is characterized by needle-like, serrated protuberance, outer membrane and myoid zone. The ellipsoid zone showed unclear or disappeared and superficial retinal detachment. The above features recovered well after early regular treatment.
