静态姿势图在良性阵发性位置性眩晕中感觉结构变化研究

【摘要】：目的通过静态姿势图可分析BPPV患者感觉结构(Sensory structure)变化的特征;明确BPPV患者康复训练后感觉结构变化情况;方法选取符合诊断标准的BPPV患者,采用静态姿势仪进行感觉结构变化评估前庭康复,定量评价眩晕患者在消除或减弱视觉和本体觉的情况下前庭功能及半规管功能,并采用姿势图分析前庭功能低下时视觉前庭,脑干小脑,和脊髓本体觉的能量分布变化;结果静态姿势图可评估BPPV患者发病时在消除或减弱视觉系统和本体觉系统功能时,明确前庭系统中半规管功能低下的情况。;BPPV患者发病时要维持平衡,则需要通过视觉或本体觉加强,从而发现眩晕患者维持平衡很多程度依靠视觉和小脑脑干平衡功能;运用前庭康复系统按要求对外周性眩晕患者进行平衡康复训练,而且前庭功能测试后前庭评分恢复正常且显著高于对照组(P0.05);结论 BPPV患者大部分出现前庭系统功能评分低,并能发现前庭半规管功能低下时评分低的半规管侧别;评价BPPV患者在不同状态下患者维持平衡很多程度依靠视觉和小脑平衡功能;前庭康复对BPPV患者疗效明确。
[Abstract]:Objective to analyze the changes of sensory structure in patients with BPPV by static posturography, to determine the changes of sensory structure after rehabilitation training in patients with BPPV, and to select BPPV patients who meet the diagnostic criteria. The vestibular rehabilitation was evaluated with static postural apparatus. The vestibular function and semicircular canal function were quantitatively evaluated under the condition of eliminating or weakening visual and proprioceptive perception in vertigo patients, and the visual vestibular function was analyzed with postural map when the vestibular function was low. Results static posturography could be used to evaluate the function of visual system and proprioceptive system in patients with BPPV in the process of eliminating or weakening the function of visual system and proprioceptive system. In order to maintain the balance during the onset of BPPV, it is necessary to strengthen the visual or proprioceptive perception, thus finding that the maintenance of balance in vertigo patients depends on visual and cerebellar brainstem balance to a great extent. The vestibular rehabilitation system was used for balanced rehabilitation training in patients with peripheral vertigo, and the vestibular score recovered to normal after the vestibular function test and was significantly higher than that in the control group (P 0.05). Conclusion the vestibular system function score is low in most patients with BPPV. We can find the semicircular canal side with low vestibular semicircular canal function; evaluate the balance of patients with BPPV in different state depends on visual and cerebellar balance function; vestibular rehabilitation is effective for BPPV patients.
