西宁地区OSAHS与高血压的关系探讨

【摘要】:目的:探讨西宁地区阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与高血压的关系及OSAHS相关高血压的发病机制。 方法:选择西宁地区33例OSAHS患者作为研究对象,均为男性,平均年龄46.91±9.03岁;20例健康人群作为健康对照组,均为男性,年龄40.80±12.14岁。两组人员均在行多导联睡眠监护前及监护后测定血压,并将OSAHS患者的血压与各睡眠呼吸参数进行相关分析。 结果:1)西宁地区OSAHS组体重指数(BMI)较正常组明显升高(P0.05);2)西宁地区OSAHS组呼吸暂停最长时间、呼吸暂停指数(AHI)、氧饱和度(SaO_2)85%时间均明显高于对照组(P0.05),而OSAHS组的平均氧饱和度(MSaO_2)、最低氧饱和度(LSaO_2)均明显低于对照组(P0.05);3)西宁地区OSAHS组的睡前及醒后血压均高于对照组(P0.05);4)西宁地区OSAHS组的收缩压(SBP)、舒张压(DBP)与AHI呈正相关(P0.05),与LSaO_2呈负相关(P0.05)。 结论:西宁地区OSAHS组的SBP、DBP与AHI呈正相关,与LSaO_2呈负相关,且OSAHS组LSaO_2较对照组明显降低,提示OSAHS引起的夜间低氧血症可能是高血压病因之一,低压低氧环境可能会进一步加重高血压的发生。
[Abstract]:Objective: to investigate the relationship between obstructive sleep apnea hypopnea syndrome (OSAHS) and hypertension and the pathogenesis of OSAHS related hypertension in Xining area. Methods: 33 patients with OSAHS in Xining area were selected as study subjects, all of them were male, and 20 healthy people (mean age 46.91 卤9.03 years old) were selected as healthy control group, all of them were male. The age was 40.80 卤12.14 years old. The blood pressure was measured before and after polysomnography in both groups, and the correlation between blood pressure and sleep breathing parameters in OSAHS patients was analyzed. Results the body mass index (BMI) of the OSAHS group in Xining area was significantly higher than that of the normal group (P 0.05). The longest time of apnea was found in the OSAHS group in Xining area. Apnea index (AHI), oxygen saturation (SAO _ 2 / 85%) were significantly higher than those in control group (P 0.05), while the mean oxygen saturation (MSaO _ 2) and lowest oxygen saturation (L _ SaO _ 2) in OSAHS group were significantly lower than those in control group (P < 0.05). The blood pressure before sleep and after waking in OSAHS group in Xining area was higher than that in control group. The systolic blood pressure (SBP) and diastolic blood pressure (DBP) of OSAHS group in Xining area were positively correlated with AHI (P 0.05) and negatively correlated with LSaO_2 (P 0.05). Conclusion: SBP AHI in OSAHS group in Xining area was positively correlated with AHI and negatively correlated with LSaO_2, and LSaO_2 in OSAHS group was significantly lower than that in control group, suggesting that OSAHS induced nocturnal hypoxemia may be one of the causes of hypertension. Low-pressure hypoxia may further aggravate the occurrence of high blood pressure.
