【摘要】:【目的】观察比较经巩膜瓣下穿刺口置入Ahmed青光眼阀(AGV)与经巩膜长隧道置入AGV治疗新生血管性青光眼(NVG)的疗效及并发症等。 【方法】将20例(20只眼)NVG患者随机均分为二组,分别经巩膜瓣下穿刺口及经巩膜长隧道置入AGV,术后3d,7d,1m,3m,6m,12m检查视力、眼压及裂隙灯检查等。 【结果】随防1~29个月,平均11.6个月。巩膜瓣组患者末次随访眼压由术前(49.7±8.33)mmHg降至(17.0±8.97)mmHg,手术前后眼压差异有统计学意义(P0.05);巩膜长隧道组患者末次随访眼压由术前(46.2±7.04)mmHg降至(18.5±7.78)mmHg,,手术前后眼压差异有统计学意义(P0.05);二组患者术后眼压相较差异无统计学意义(P0.05)。巩膜长隧道组患者低眼压性浅前房的发生率较巩膜瓣组低,其差异具统计学意义(P0.05),其余并发症的发生率,如前房积血,高眼压等,二组相较无统计学意义(P0.05)。 【结论】通过二种途径置入AGV治疗NVG降低眼压效果相当,但经巩膜长隧道置入其操作更简便、局部组织损伤更轻、材料成本更低、术后早期低眼压性浅前房的发生率更少,或可成为房水引流物置入的新方法。
[Abstract]:[objective] to compare the curative effect and complications of Ahmed glaucoma valve implantation via subscleral valve and AGV implantation through long scleral tunnel in the treatment of neovascular glaucoma. [methods] Twenty patients with NVG were randomly divided into two groups: subscleral puncture orifice and transscleral long tunnel were implanted into AGV.The visual acuity, intraocular pressure and slit lamp were examined 3 days after operation. [results] after 1 ~ 29 months of prevention, The IOP of scleral flap group decreased from 49.7 卤8.33)mmHg to 17.0 卤8.97mm Hg, and the IOP of scleral tunnel group decreased from 46.2 卤7.04)mmHg to 18.5 卤7.78mmHg.The IOP of scleral long tunnel group decreased from 46.2 卤7.04)mmHg to 18.5 卤7.78mmHg before and after operation. There was no significant difference in IOP between the two groups. The incidence of hypotony shallow anterior chamber in scleral long tunnel group was lower than that in scleral flap group. The difference was statistically significant (P 0.05). The incidence of other complications, such as hyphema and high intraocular pressure, was not statistically significant in the two groups. [conclusion] the effect of AGV implantation in the treatment of NVG is comparable, but the operation of long tunnel through sclera is easier, the local tissue injury is lighter, the cost of materials is lower, and the incidence of early low IOP shallow anterior chamber is less. It may be a new method for drainage of aqueous humor.
