OSAHS患者血清外周血干细胞生长因子和髓过氧化物酶的表达

【摘要】：目的:探讨OSAHS患者外周血干细胞生长因子(HGF)、髓过氧化物酶(MPO)的表达及意义。方法:通过PSG确诊的96例OSAHS患者为OSAHS组,根据AHI范围分为重、中、轻3组;32例于我科体检健康者为对照组;ELISA法测定血HGF、MPO水平;予以重度OSAHS患者手术+CPAP方案治疗3个月并复测体内HGF、MPO浓度,分析HGF、MPO和PSG指标的相关性。结果:(1)随着OSAHS病情程度增加,其外周血HGF和MPO浓度逐渐上升,不同程度各组患者HGF、MPO不全相同(F=119.006,25.138;P0.01);其中轻度组与对照组间无明显差别(P0.05);其余各组相互比较均有明显差别(P0.05)。(2)OSAHS患者血HGF、MPO浓度和年龄、BMI无关(P0.05),而HGF与MPO浓度改变呈正相关,且二者均与AHI呈正相关,与LSaO2呈负相关(P0.05)。(3)32例重度OSAHS患者经过3个月综合方案治疗后LSaO2上升,AHI、外周血HGF、MPO水平降低,均有明显差别(P0.01)。治疗6个月后MPO、HGF浓度较治疗3个月时降低(P0.05),但和对照组相比仍有升高(P0.05)。结论:手术联合较长时程CPAP治疗可降低OSAHS患者缺氧程度,降低心血管损害,其疗效和疗程相关;联合测定OSAHS患者外周血HGF与MPO对病情、疗效的判断及并发心血管损害的评估有一定的临床参考价值。
[Abstract]:Objective: to investigate the expression and significance of peripheral blood stem cell growth factor (OSAHS) in patients with OSAHS. Methods: 96 patients with OSAHS diagnosed by PSG were divided into two groups: OSAHS group according to the range of AHI. The serum levels of HGF MPO were measured by Elisa in 32 healthy subjects in our department, and the levels of HGFG MPO in patients with severe OSAHS were measured by CPAP regimen for 3 months. The correlation between MPO and PSG was analyzed. Results: with the increase of the severity of OSAHS, the HGF and MPO concentrations in peripheral blood increased gradually. There was no significant difference between mild group and control group (P 0.05), and there was significant difference between other groups (P 0.05). There was no significant correlation between HGF and MPO. Both of them were positively correlated with AHI, and negatively correlated with LSaO2 in 32 patients with severe OSAHS. After 3 months of comprehensive treatment, LSaO2 increased and the level of MPO in peripheral blood decreased. After 6 months of treatment, the concentration of MPOG was lower than that at 3 months, but it was still higher than that of the control group. Conclusion: operation combined with long-term CPAP treatment can reduce the degree of hypoxia and cardiovascular damage in patients with OSAHS. The combined determination of HGF and MPO in peripheral blood of patients with OSAHS has a certain clinical reference value in the evaluation of disease, curative effect and cardiovascular damage.
