白内障两种手术切口对角膜散光影响的比较

【摘要】：目的 比较白内障超声乳化吸除联合人工晶状体植入术两种手术切口方式对角膜散光的影响。 方法 将62例(76只眼)老年性白内障患者随机分为A、B两组,行白内障透明角膜切口超声乳化吸除及人工晶状体植入术,其中A组患者术中均在颞上方(右眼)、鼻上方(左眼)行3mm透明角膜切口,B组患者根据术前角膜曲率计检查选择角膜屈光力最大子午线方向行3mm透明角膜切口,观察比较术后角膜散光及视力情况。 结果 统计学分析显示,两组患者术后裸眼视力与术前相比较差异显著；A组中术前角膜散光与术后3天、1月、3月时无明显变化,B组中术前角膜散光度1.0D的患者术后3d、1月、3月角膜散光与术前比较差异无统计学意义,但B组中术前角膜散光度≥1.0D的患者术后3d、1月、3月角膜散光与术前比较差异有统计学意义。 结论 对术前角膜散光1.0D者,白内障超声乳化术中行3mm透明角膜切口,对其不产生影响；但对于术前角膜散光度数≥1.0D的患者,术中在角膜屈光率最大子午线方向行3mm透明角膜切口可减少部分角膜散光,且有望能使患者获得更好的裸眼视力。
[Abstract]:PurposeTo compare the effect of phacoemulsification and intraocular lens implantation on corneal astigmatism.Method62 cases (76 eyes) of senile cataract were randomly divided into two groups: group A and group B: phacoemulsification with transparent corneal incision and intraocular lens implantation.In group A, 3mm transparent corneal incision was performed at the superior temporal (right eye) and the upper nasal (left eye). According to the preoperative keratometer examination, the 3mm transparent corneal incision was performed in the direction of the largest meridian of corneal refractive power.Corneal astigmatism and visual acuity were observed and compared.ResultStatistical analysis shows thatThere was no statistical difference between the two groups.But in group B, corneal astigmatism with preoperative astigmatism 鈮，
