新生儿听力筛查中联合应用DPOAE和AABR的回顾性分析

【摘要】:目的 :初步分析畸变产物耳声发射(distortion product otoacoustic emission,DPOAE)联合自动听性脑干反应(automated auditory brainstem response,AABR)在新生儿听力筛查中的应用。方法:回顾性分析2012年5月—2013年5月新生儿在本院听力筛查结果,新生儿出生后3~4 d进行DPOAE初筛,出生后42 d进行复筛,进行复筛时,初筛通过者应用AABR检查,未通过者复查DPOAE的同时进行AARB检查。复筛中有任一项检查未通过者接受听力学诊断评估,包括听性脑干反应测试技术(auditory brainstem response,ABR)和声导抗检查。结果:共7 569例进行了DPOAE和AABR的联合筛查,其中有191例进行了ABR检查,AABR与ABR的一致率为91.62%,DPOAE与ABR的一致率为57.07%,而DPOAE联合AABR筛查与ABR的一致率为66.67%。在191例ABR检查中127例(213耳)存在听力异常,其中轻度听力损失72耳(33.80%),中度听力损失54耳(25.35%),重度听力损失25耳(11.74%),极重度听力损失62耳(29.11%)。结论:联合应用DPOAE和AABR可以降低新生儿听力异常的漏诊、误诊率,对于听力障碍患儿的早期发现、早期诊断、早期治疗具有重要的意义。
[Abstract]:Objective: to analyze the application of distortion product otoacoustic emission (distortion product otoacoustic emission) combined with automatic auditory brainstem response (AABR) in neonatal hearing screening.Methods: the results of neonatal hearing screening in our hospital from May 2012 to May 2013 were retrospectively analyzed. DPOAE screening was performed 3 days after birth and 42 days after birth. When screening was performed, AABR examination was applied to those who passed the screening.AARB was performed at the same time as DPOAE was rechecked by those who failed.Any of the patients who failed the screening were assessed for audiological diagnosis, including auditory brainstem response (ABR) and acoustic impedance (ABR).缁撴灉:鍏,
