鼻咽癌患者放疗前后垂体激素水平的变化

【摘要】：目的：通过测定鼻咽癌患者放疗前后垂体激素水平,探讨放疗对鼻咽癌(NPC)患者的垂体激素水平的影响。方法：从2003年8月至2011年8月,在浙江大学附属二院肿瘤放疗科一个治疗小组选择101例鼻咽癌患者,行回顾性描述性研究。对这101例鼻咽癌患者,行鼻咽部及颈部淋巴结放疗前及放疗结束时、放疗后数月至数年间,复查其血清中高敏促甲状腺激素(h-TSH)、生长激素(GH)、催乳素(PRL)、黄体生成素(LH)、卵泡刺激素(FSH)的浓度,通过统计分析,探讨放疗对鼻咽癌(NPC)患者的垂体激素水平的是否有影响,对何种垂体激素影响更明显,何时出现最明显的影响等。结果：总病例共101例,其中男性64例,女性37例,男女比例为：1.81：1.年龄在16-83岁之间,中位年龄为49岁。对101例患者放疗前及放疗结束时、放疗后数月至数年间,复查其血清中生长激素(GH)、催乳素(PRL)、黄体生成素(LH)、卵泡刺激素(FSH)及促甲状腺激素(TSH)的浓度,从放疗前开始入组,最长随访至放疗后73月。结果显示高敏促甲状腺激素(h-TSH)在放疗后4月-4年间激素水平变化存在统计学差异(P0.05)；生长激素(GH)在放疗后1-3月及7-12月较放疗前激素水平变化存在统计学差异(P0.05)；男性催乳素(PRL)在放疗后7-12月较放疗前激素水平变化存在统计学差异(P0.05)；男性黄体生成素(LH)在放疗后4-6月较放疗前激素水平变化存在统计学差异(P0.05),而在放疗后19-24月内较放疗结束时黄体生成素(LH)激素水平仍有统计学差异(P0.05)；男性卵泡刺激素(FSH)在放疗后3年内较放疗前激素水平变化有统计学差异(P0.05)。结论：鼻咽癌患者放疗前后垂体激素水平发生了动态的变化。
[Abstract]:Objective:To investigate the effect of radiotherapy on pituitary hormone levels in patients with nasopharyngeal carcinoma (NPC) before and after radiotherapy.Methods:From August 2003 to August 2011, a retrospective descriptive study was conducted in 101 patients with nasopharyngeal carcinoma in the Department of tumor radiotherapy, second affiliated Hospital of Zhejiang University.For these 101 patients with nasopharyngeal carcinoma, the nasopharyngeal and cervical lymph nodes were treated months to years after radiotherapy, before and at the end of radiotherapy.To investigate the effects of radiotherapy on pituitary hormone levels in patients with nasopharyngeal carcinoma (NPC), the serum concentrations of Gao Min TSH, GH, PRL, LHN and FSHS were reviewed, and the effects of radiotherapy on pituitary hormone levels in patients with nasopharyngeal carcinoma (NPC) were investigated by statistical analysis.To which pituitary hormone influence is more obvious, when appears the most obvious influence and so on.Results:The total number of cases was 101, including 64 males and 37 females, with a ratio of 1. 81 to 1. 1.The age ranged from 16 to 83 years, with a median age of 49 years.The serum concentrations of growth hormone GHG, prolactin prolactin, luteinizing hormone (LHH), follicle stimulating hormone (FSH) and thyroid stimulating hormone (TSH) were examined in 101 patients before and after radiotherapy for months to several years after radiotherapy.Up to 73 months after radiotherapy.The results showed that there were significant differences in hormone levels between 4 and 4 months after radiotherapy (P 0.05) and between January and July in July and December after radiotherapy. There were significant differences in hormone levels between January and January and July and December after radiotherapy in males; in males, there were significant differences in hormone levels between 3 months and 7 months after radiotherapy.Prolactin prolactin (PRL) levels were significantly different from those before radiotherapy in July and 12 months after radiotherapy (P 0.05), and in male luteinizing hormone (LH) from April to June after radiotherapy compared with those before radiotherapy (P 0.05), and within 19 to 24 months after radiotherapy, there was a significant difference in the changes of hormone levels in male luteinizing hormone (LH).At the end of the therapy, there was still a statistical difference in the level of luteinizing hormone (LH) and the changes of male follicle stimulating hormone (FSH) in three years after radiotherapy compared with that before radiotherapy (P 0.05).Conclusion:Pituitary hormone levels in nasopharyngeal carcinoma patients changed dynamically before and after radiotherapy.
