眼科OCT与FFA观察糖尿病性视网膜病变黄斑改变

【摘要】:目的本研究的目的在于应用光学相干断层扫描(OCT)以及眼底荧光血管造影(FFA),作为两种客观的检查手段观察糖尿病性视网膜病变(DR)在各个阶段的黄斑改变,探讨DR年龄、性别、糖尿病病程对黄斑区的影响。方法应用蔡司3代时域光学相干断层扫描(OCT)OCTA 6.01软件版本分析,应用重庆康华眼底荧光造影(FFA),对123例246只眼DR患者进行检查,并按年龄、性别及病程年限分组。结果123例246只眼DR患者中,20例40只眼,年龄25~85岁,男14例,女6例,病程1~10年,OCT观察无明显糖尿病性黄斑病变,FFA后极部无明显视网膜增厚或硬性渗出。36例72只眼,年龄31~81岁,男16例,女20例,病程1~30年,OCT观察轻度糖尿病性黄斑病变,FFA观察后极部存在部分视网膜增厚或硬性渗出,但远离黄斑中心。32例64只眼,年龄38~78岁,男10例,女22例,糖尿病史4~30年,OCT观察中度糖尿病性黄斑,FFA观察视网膜增厚或硬性渗出接近黄斑但未涉及黄斑中心。35例70只眼,年龄27~76岁,男11例,女24例,糖尿病史3~32年,OCT观察重度糖尿病性黄斑病变,FFA观察视网膜增厚或硬性渗出涉及黄斑中心。结论 DR在各个阶段,其中,发病8~20年间、年龄45~65岁之间、性别间,差异无统计学意义,DR发展成视网膜病变、黄斑改变等增殖性糖尿病性视网膜病变(PDR)的几率更高,病情发展更快,同时进一步证实FFA与OCT联合检查,对于判断DR病变,发展阶段及严重程度至关重要。
[Abstract]:Objective to investigate the macular changes of diabetic retinopathy in different stages by using optical coherence tomography (Oct) and fundus fluorescein angiography (FFAA), and to investigate the age and sex of Dr.The effect of the course of diabetes on macular area.Methods A software version of OCTA 6.01 was used to analyze OCTA 6.01 of Zeiss' 3rd generation optical coherence tomography. 123 patients (246 eyes) with Dr were examined by FFAA of Chongqing Kanghua fundus fluorescein angiography. The patients were divided into three groups according to age, sex and duration of disease.Results among the 246 eyes of 123 patients with Dr, 20 eyes (40 eyes), aged 2585 years, 14 males and 6 females, underwent Oct for 1 ~ 10 years. There was no obvious retinal thickening or hard exudation after FFA in 72 eyes of 36 patients (age 31: 81).16 males and 20 females were observed by Oct for 1 ~ 30 years. Partial retinal thickening or rigid exudation was observed in the posterior pole of FFA. However, 64 eyes were located far away from the center of macula in 64 eyes, aged 38 ~ 78 years, 10 males and 22 females.Oct from 4 to 30 years of history of diabetes, FFA of moderate diabetic macular macula was observed in 70 eyes of 35 cases (age 2776 years) with retinal thickening or rigid exudation close to macula but not involved in the center of macula. There were 11 males and 24 females.Observation of severe diabetic macular lesions with Oct for 3 ~ 32 years FFA was used to observe retinal thickening or rigid exudation involving macular center.Conclusion Dr is more likely to develop into retinopathy, such as macular change, proliferative diabetic retinopathy (PDR) in 820 years, between 45 and 65 years of age, and there is no significant difference between the sexes.At the same time, the combined examination of FFA and OCT is very important to judge the pathological changes, development stage and severity of Dr.
