不同类型、不同程度弱视对立体视觉的影响

【摘要】:目的:观察矫正视力到达1.0时弱视患者的远、近立体视觉,探讨不同类型、不同程度弱视对立体视觉的影响。方法:回顾性分析2014年10月至2016年12月双眼视力矫正均达1.0弱视患者共247例,屈光不正性弱视116例,屈光参差性弱视84例,斜视性弱视30例,形觉剥夺性弱视17例。根据初诊时矫正视力分为轻度、中度、重度三组。采用颜少明立体视觉图测定近立体视,近立体视锐度按照15″~60″、80″~200″、400″~3000″分为中心立体视、黄斑立体视、周边立体视三组,统计各类型、不同程度中心立体视、黄斑立体视、周边立体视人数,比较不同类型、不同程度弱视在三组中近立体视锐度有无统计学意义。同视机测定远立体视,按照存在、不存在,分别记为阳性(+)、阴性(-)两组,统计各类型、不同程度弱视两组人数,比较不同类型、不同程度弱视远立体视觉有无统计学意义。结果:(1)不同类型与立体视觉的关系:屈光不正性弱视中心立体视锐度存在率最高,占60.34%,屈光参差性弱视中心立体视锐度占47.61%,斜视性弱视中心立体视锐度占26.67%,形觉剥夺性弱视中心立体视锐度占29.41%。斜视性弱视周边立体视锐度存在率最高,占33.33%,形觉剥夺性弱视周边立体视锐度占29.41%,屈光参差性弱视周边立体视锐度占15.48%,屈光不正性弱视周边立体视锐度占8.62%。4种类型弱视近立体视比较有统计学意义(Z=22.48,P0.05)。4种类型弱视远立视觉比较无统计学意义(c2=2.939,P0.05)。(2)各类型中不同程度与立体视觉的关系:屈光不正性弱视中轻度弱视中心立体视锐度存在率最高,占59.46%,中度弱视中心立体视锐度占40.00%,重度弱视中心立体视锐度占17.24%,重度弱视周边立体视视锐度存在率高,占62.07%,中度弱视周边立体视锐度占40.00%,轻度弱视患者周边立体视锐度占18.92%。3种程度弱视近立体觉比较有统计学意义(Z=14.84,P0.05)。3种程度远立体视觉比较无统计学意义(c2=3.024,P0.05)。屈光参差性弱视中轻度弱视中心立体视锐度存在率最高,占66.67%,中度弱视中心立体视锐度占32.35%,重度弱视中心立体视锐度占25.00%,重度弱视周边立体视视锐度存在率高,占50.00%,中度弱视周边立体视锐度占14.70%,轻度弱视患者周边立体视锐度占16.67%。3种程度弱视近立体觉比较有统计学意义(Z=-2.970,P0.05)。3种程度远立体视觉比较无统计学意义(c2=0.272,P0.05)。结论:(1)不同类型、不同程度弱视对近立体视影响不同,而远立体视无明显差异。(2)屈光不正性弱视近立体视觉恢复最好,其次是屈光参差性弱视,斜视性弱视、形觉剥夺性弱视主要为周边立体视。(3)弱视程度越轻,近立体视恢复越好。
[Abstract]:Objective: to observe the effects of different types and degrees of amblyopia on stereopsis in patients with amblyopia when corrected visual acuity reaches 1.0. Methods: from October 2014 to December 2016, 247 patients with 1.0 amblyopia, 116 ametropia amblyopia, 84 anisometropia amblyopia, 30 strabismus amblyopia and 17 form deprivation amblyopia were analyzed retrospectively. According to the corrected visual acuity at first visit, it was divided into three groups: mild, moderate and severe. The near stereopsis was measured by Yan Shaoming stereovision. The near stereopsis acuity was divided into three groups: central stereopsis, macular stereopsis and peripheral stereopsis. The number of macular stereopsis, peripheral stereopsis, different types and different degrees of amblyopia were statistically significant in the three groups. According to the existence and non-existence of distant stereopsis, the same vision machine was recorded as positive group (P < 0.01) and negative group (P < 0.01). The number of people in each type and different degree amblyopia group were counted, and there was statistical significance in comparing different types and different degrees of amblyopia far stereoscopic vision. Results the relationship between different types of stereopsis and stereopsis: ametropia amblyopia center had the highest rate of stereopsis acuity. The stereoscopic acuity of anisometropia amblyopia center was 47.61, that of strabismus amblyopia center was 26.67, and that of form deprivation amblyopia center was 29.41. The existence rate of stereopsis acuity around strabismus amblyopia was the highest. Accounting for 33.33, the peripheral stereopsis acuity of form deprivation amblyopia is 29.41, that of anisometropia amblyopia is 15.48, and that of ametropia amblyopia peripheral stereopsis is 8.62.4 kinds of amblyopia near stereopsis have statistical significance. There was no statistical significance between the visual acuity of amblyopia and the relationship between different degrees and stereopsis in different types of amblyopic amblyopia: in the amblyopic amblyopia with mild amblyopia, the central stereopsis acuity of mild amblyopia was the highest, and the visual acuity of the amblyopia center was the highest in all types of amblyopia. The central stereopsis acuity of moderate amblyopia center was 40.00.The stereopsis acuity of severe amblyopia center was 17.24.The stereopsis acuity around severe amblyopia was high. There were significant differences in the degree of peripheral stereopsis between moderate amblyopia and mild amblyopia (18.92.3). There was statistical significance in the comparison of the near stereopsis of amblyopia. There was no statistical significance in the comparison of the degree of stereoscopic acuity between moderate amblyopia and mild amblyopia (P 0.053.024P0.05). In anisometropic amblyopia, the rate of central stereopsis acuity of mild amblyopia was the highest (66.67%), that of moderate amblyopia center was 32.35%, that of severe amblyopia center was 25.00%, that of severe amblyopia peripheral stereopsis was high, that of moderate amblyopia center was 32.35%, and that of severe amblyopia center was 25.00%. There were significant differences in the peripheral stereopsis acuity between moderate amblyopia and mild amblyopia patients (16.67.3 and 16.67.3, respectively). There was no statistical significance between the degree of amblyopia and the near stereopsis of amblyopia. There was no significant difference in the degree of stereopsis between moderate amblyopia and mild amblyopia. Conclusion (1) different types of amblyopia have different effects on near stereopsis, but there is no obvious difference in far stereopsis. The amblyopia of ametropia amblyopia is the best, followed by anisometropia amblyopia and strabismus amblyopia. The less degree of amblyopia, the better the recovery of near stereopsis.
