S100A4、MMP-2和MMP-9在鼻咽癌中的表达及其临床病理学意义

【摘要】:目的:研究S100A4、MMP-2和MMP-9在鼻咽癌组织中的表达及其与鼻咽癌临床病理参数的关系及意义。 方法:采用免疫组化S-P法检测临床病理完整的72例鼻咽癌中S100A4、MMP-2和MMP-9蛋白的表达,并采用χ2检验分析S100A4、MMP-2和MMP-9与临床病理因素的关系采用Kaplan-Meier法进行生存分析,log-rank法检验差异性,应用spearman等级分析三者之间的关系。 结果:(1)S100A4在鼻咽癌组织中的阳性表达率(76.4%)明显高于鼻咽慢性炎(15%),差异具有统计学意义(P0.05);MMP-2在鼻咽癌组织中的阳性表达率(69.4%)明显高于鼻咽慢性炎(0%),差异有统计学意义(P0.05);MMP-9在鼻咽癌组织中的阳性表达率(77.8%)明显高于鼻咽慢性炎(0%),差异具有统计学意义(P0.05)。(2)72例鼻咽癌中,S100A4在淋巴结转移中,N2~3期中S100A4蛋白的表达明显高于N0~1期(P<0.05);在临床分期中,Ⅲ~Ⅳ期表达水平明显高于Ⅰ~Ⅱ期,差异有显著性;有颅底侵犯的表达水平明显高于无颅底侵犯(P<0.05);MMP-2蛋白在淋巴结分期中,N2~3期中的表达明显高于N0~1(P<0.05);临床分期中,Ⅲ~Ⅳ期明显高于Ⅰ~Ⅱ期,差异有显著性(P<0.05);MMP-9蛋白在淋巴结分期中,N2~3期中的表达明显高于N0~1(P<0.05);在临床分期中,Ⅲ~Ⅳ期明显高于Ⅰ~Ⅱ期,差异有显著性(P<0.05)。(3)S100A4阳性组中位生存时间为47月(95%CI:43.8,50.2),5年生存率为36.4%,而S100A4阴性组中位生存时间为58月(95%CI:55.1,60.9),5年生存率为52.9%(p<0.05);MMP-2阳性组中位生存时间为47月(95%CI:44.7,49.3),5年生存率为34%,而MMP-2阴性组中位生存时间为58月(95%CI:53.7,62.3),5年生存率为54.5%(p<0.05);MMP-9阳性组中位生存时间为48月(95%CI:46.1,49.9),5年生存率为34%;而MMP-9阴性组中位生存时间为58月(95%CI:51.2,64.8),5年生存率为54.5%,,(p<0.05)。(4)S100A4表达阳性的55例中,MMP-2表达阳性的有47例,表达阴性的有8例,两者呈正相关性(P=0.000,r=0.625)。S100A4表达阳性55例中,MMP-9表达阳性的有49例、表达阴性的有6例,两者呈正相关性(P=0.000,r=0.489)。MMP-2与MMP-9的表达无相关性(P0.05) 结论:(1)S100A4、MMP-2和MMP-9蛋白在鼻咽癌中的表达可作为鼻咽癌恶性程度的参考指标;S100A4、MMP-2和MMP-9蛋白异常表达与鼻咽癌发生有关;(2)S100A4、MMP-2和MMP-9的过表达与鼻咽癌的淋巴结转移及临床分期相关,S100A4的表达与鼻咽癌的浸润生长相关;(3)S100A4、MMP-2和MMP-9阳性表达均与不良预后相关;(4)S100A4与MMP-2、MMP-9表达呈正相关性,MMP-2与MMP-9的表达无相关性。
[Abstract]:Objective: to study the expression of S100A4 mMMP-2 and MMP-9 in nasopharyngeal carcinoma (NPC) and its relationship with clinicopathological parameters. Methods: immunohistochemical S-P method was used to detect the expression of S100A4mpMMP-2 and MMP-9 protein in 72 cases of nasopharyngeal carcinoma with clinicopathologic integrity. The relationship between S100A4mpMMP-2 and MMP-9 and clinicopathological factors was analyzed by Kaplan-Meier method. The relationship between the three is analyzed by using spearman grade. Results the positive expression rate of S100A4 in nasopharyngeal carcinoma (76. 4) was significantly higher than that of chronic nasopharyngitis (P 0. 05). The positive rate of MMP-2 in nasopharyngeal carcinoma was significantly higher than that in nasopharyngeal carcinoma (P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05). The positive expression rate of S100A4 in nasopharyngeal carcinoma (77.8%) was significantly higher than that in chronic nasopharyngitis (P 0.05). The expression of S100A4 protein in lymph node metastasis of 72 cases of nasopharyngeal carcinoma was significantly higher than that of N0 stage 1 (P < 0.05), and the expression of S100A4 was significantly higher than that of N0 stage 1 (P < 0.05), and the expression of S100A4 protein in lymph node metastasis was significantly higher in nasopharyngeal carcinoma (P < 0.05) than that in nasopharyngeal carcinoma (P < 0.05).
