喉不同区域淋巴示踪剂流向分布的实验研究

【摘要】:[目的]:通过将淋巴结示踪剂纳米炭、亚甲蓝、中华墨汁注射入活体兔喉腔内声门及声门上区,动态观察其在兔喉部、颈部淋巴结染色情况,依此比较上述三种淋巴结示踪剂在喉腔不同部位给药条件下颈部淋巴结染色示踪效果,为日后相关淋巴结示踪剂在喉癌及其淋巴结转移的患者中的临床应用提供理论和实验依据。[方法]:取新西兰大白兔30只,随机分成6组,每组5只,将实验兔麻醉,经口在电子喉镜引导下使用喉针在兔声门上部、声门部分别注射纳米炭、亚甲蓝、中华墨汁。退出喉镜后15min内完成对实验兔行颈部淋巴结的解剖,术中尽量减少对喉部淋巴管网的损伤,并随时间动态观察实验兔喉部前哨淋巴结的染色数量,开始着色时间及染色消褪时间等数据并相互比对分析。[结果]:实验组中纳米炭组染色淋巴结最多共计36枚。亚甲蓝组次之共染色33枚淋巴结,中华墨汁组最少共染色7枚淋巴结。对于声门上部注射组来说:亚甲蓝组淋巴结染色平均数为(5.400± 1.140)枚、纳米炭组为(7.400±1.140)枚,P0.05,两组有统计学意义;而着色的时间上亚甲蓝组为(20.400±1.673)min,纳米炭组为(31.200±2.864)min,两组比较有统计学意义(P0.05)。对于声门部注射组来说:亚甲蓝组染色淋巴结为(1.200±0.447)枚,纳米炭组为(1.800±0.837)枚,两组无明显统计学意义(P=0.305);着色的时间上亚甲蓝组为(102.200±3.564)min,纳米炭组为(86.600±2.702)min,两组相比较有统计学意义(P0.05)。[结论]:纳米炭组和亚甲蓝组均能将实验兔喉部引流的前哨淋巴结染色,纳米炭组淋巴结染色数较多,着色时间上各有差异,总体而言两者各具优缺点。而中华墨汁在喉部无论是染色淋巴结数量还是着色时间上均较亚甲蓝和纳米炭两者效果差,不适合作为兔喉部淋巴结示踪剂使用。组织被3种淋巴结示踪剂上色后均不会影响标本的病理检验。
[Abstract]:[objective]: by injecting lymph node tracer carbon, methylene blue and Chinese ink into the glottis and supraglottic region of rabbit larynx in vivo, the staining of lymph nodes in rabbit larynx and neck was observed dynamically. According to the above mentioned three kinds of lymph node tracers, the effect of cervical lymph node staining on different parts of laryngeal cavity was compared. To provide theoretical and experimental basis for clinical application of lymph node tracer in patients with laryngeal carcinoma and lymph node metastasis. [methods] Thirty New Zealand white rabbits were randomly divided into 6 groups (5 rabbits in each group). The experimental rabbits were anesthetized with laryngeal needle under the guidance of electronic laryngoscope, and the glottis were injected with carbon nanoparticles, methylene blue and Chinese ink respectively under the guidance of electronic laryngoscope. After laryngoscope, the cervical lymph nodes of experimental rabbits were dissected in 15min, the injury of laryngeal lymphatic network was minimized during the operation, and the number of sentinel lymph nodes in larynx of experimental rabbits was observed dynamically over time. Start coloring time and dyeing fading time and other data and compare analysis with each other. Results: in the experimental group, 36 lymph nodes were stained. 33 lymph nodes were stained in methylene blue group and 7 lymph nodes were stained in Chinese ink group. For the upper glottic injection group, the mean number of lymph nodes staining was 5.400 卤1.140 in methylene blue group and 7.400 卤1.140 in nano-carbon group, which was statistically significant in both groups, but the staining time in methylene blue group was 20.400 卤1.673 minutes, and that in nano-carbon group was 31.200 卤2.864 minutes. For glottic injection group, the stained lymph nodes were 1.200 卤0.447 in methylene blue group and 1.800 卤0.837 in carbon nano-group. There was no significant difference between the two groups in the staining time of methylene blue group (102.200 卤3.564min) and nano-carbon group (86.600 卤2.702min). [conclusion] both nano-carbon group and methylene blue group can stain the sentinel lymph nodes draining from the larynx of experimental rabbits. The numbers of lymph nodes in the nano-carbon group are more, and the coloring time is different. Generally speaking, the two groups have their own advantages and disadvantages. Chinese ink is less effective than methylene blue and carbon nanoparticles in the number and time of lymph nodes staining in larynx, so it is not suitable to be used as tracer in rabbit laryngeal lymph nodes. Tissue coloring with three kinds of lymph node tracers did not affect the pathological examination of the specimens.
