眶脂肪弓状缘释放联合眶肌筋膜韧带悬吊与传统经皮下睑袋整形术的疗效对比

【摘要】：目的:比较眶脂肪弓状缘释放联合眶肌筋膜韧带悬吊与传统经皮入路下睑袋整形术的并发症、满意度及治疗效果,分析两者优劣。方法:选取2015年1月至2016年10月新疆医科大学第五附属医院整形外科下睑袋伴泪沟畸形受术者为研究对象,根据诊断标准、纳入标准及排除标准选取符合要求的患者共计120例,采用随机数字表分A、B两组,各计60例,其中A组为实施经皮眶脂肪弓状缘释放联合眶肌筋膜韧带悬吊术治疗下睑袋,B组为实施传统经皮下睑袋整形术。术后分别进行6~12个月的随访,比较分析两组术后的并发症、患者满意度及治疗效果,并进行统计学分析。结果:两组术后均取得良好的手术效果。两组相比,并发症下睑凹陷差异有统计学意义(P0.05),下睑外翻、球结膜水肿差异无统计学意义(P0.05)。两组术后3个月满意度调查,差异无统计学意义(P0.05),但两组在术后6个月满意度调查,差异有统计学意义(P0.05)。两组术后6个月基于Barton分级系统的泪沟畸形比较,差异有统计学意义(P0.05)。结论:眶脂肪弓状缘释放联合眶肌筋膜韧带悬吊矫正下睑袋是较为理想的手术方法,特别对于合并泪沟畸形的下睑袋,其远期效果优于传统经皮入路下睑袋整形术,患者满意度高,值得在临床中推广应用。
[Abstract]:Objective: to compare the complications, satisfaction and therapeutic effects of orbital fat arcuate release combined with suspension of orbital myofascial ligament and traditional percutaneous blepharoplasty. Methods: from January 2015 to October 2016, 120 patients with lower eyelid pouch with lacrimal sulcus malformation in plastic surgery of the Fifth affiliated Hospital of Xinjiang Medical University were selected as the subjects. 120 patients were selected according to the diagnostic criteria, inclusion criteria and exclusion criteria. 60 cases were randomly divided into two groups (n = 60). Group A was treated with percutaneous orbital fat arcuate margin release combined with suspension of orbital muscle fascia ligament for the treatment of lower eyelid pouch in group B for traditional subcutaneous blepharoplasty. The patients were followed up for 6 ~ 12 months after operation. The complications, patient satisfaction and therapeutic effect were compared and analyzed statistically. Results: good results were obtained in both groups. Compared with the two groups, the difference of lower eyelid depression in the two groups was statistically significant (P 0.05), and there was no significant difference in lower eyelid ectropion and bulbar conjunctival edema (P 0.05). There was no significant difference in the degree of satisfaction between the two groups at 3 months after operation, but there was a significant difference between the two groups in the satisfaction of the two groups at 6 months after operation (P 0.05). There was significant difference in lacrimal sulcus deformity based on Barton classification system between the two groups at 6 months postoperatively (P 0.05). Conclusion: orbital fat arcuate margin release combined with suspension of orbital myofascial ligament is an ideal method for the correction of lower eyelid pouch, especially for lower eyelid pouch with lacrimal sulcus malformation, its long-term effect is superior to that of traditional percutaneous blepharoplasty. Patient satisfaction is high and worth popularizing in clinic.
