62例儿童特发性视神经炎的临床分析

【摘要】：目的探讨儿童特发性视神经炎的临床特点,评估鼠神经生长因子(NGF)联合甲基强的松龙冲击治疗儿童特发性视神经炎的疗效。方法回顾性分析2014年1月~2015年4月我院门诊及病房确诊的62例(62只眼)儿童特发性视神经炎患儿的临床资料及随访资料。结果儿童视神经炎发病急,视力下降快,视野、VEP均有不同程度损害。NGF联合甲基强的松龙冲击治疗(治疗组)及单纯甲基强的松龙冲击治疗(对照组)后患眼的视力、视野、P-VEP均有不同程度改善。治疗一周后治疗组总有效率为93.55%,对照组为80.65%,治疗组疗效优于对照组(P0.05);治疗3个月后,两组的总有效率无明显差异(P0.05)。治疗前后两组的神经纤维层厚度均变薄,颞侧和鼻侧尤为明显(P0.05)。结论对于儿童特发性视神经炎,早期联合应用鼠神经生长因子和甲基强的松龙冲击治疗是一种有效的方法。VEP和OCT在该病的临床研究中有重要的应用价值。
[Abstract]:Objective to investigate the clinical features of idiopathic optic neuritis in children and to evaluate the efficacy of NGF combined with methylprednisolone in the treatment of idiopathic optic neuritis in children. Methods the clinical data and follow-up data of 62 children with idiopathic optic neuritis from January 2014 to April 2015 were retrospectively analyzed. Results the visual acuity of children with optic neuritis was acute, visual acuity was decreased rapidly, visual field VEP was damaged in different degree. NGF combined with methylprednisolone shock therapy (treatment group) and simple methylprednisolone impact therapy (control group). Visual field P-VEP was improved to some extent. One week after treatment, the total effective rate of the treatment group was 93.55 and that of the control group was 80.65. The curative effect of the treatment group was better than that of the control group (P 0.05), and after 3 months of treatment, there was no significant difference in the total effective rate between the two groups (P 0.05). Before and after treatment, the thickness of nerve fiber layer in both groups became thinner, especially in temporal side and nasal side. Conclusion early combination of nerve growth factor and methylprednisolone is an effective method for the treatment of idiopathic optic neuritis in children. VEP and Oct have important application value in the clinical study of the disease.
