不同疗程舌下特异性免疫治疗过敏性哮喘伴鼻炎儿童的疗效与安全性评估

【摘要】：目的:评估不同疗程标准化粉尘螨滴剂舌下含服特异性免疫治疗(SLIT)儿童过敏性哮喘(AS)伴鼻炎的疗效和安全性。方法:回顾性分析158例4~14岁AS伴鼻炎患儿接受粉尘螨滴剂舌下免疫治疗的临床指标和病例资料,根据SLIT疗程的长短被分成4组(1年、2年、3年和4年组),接受定期的临床观察及随访。评估4组患儿治疗前后的哮喘日间、夜间症状评分、哮喘用药评分、鼻炎症状评分和鼻炎用药评分的变化。同时记录治疗期间出现的不良反应。结果:与治疗前相比,1年、2年、3年以及4年疗程患儿的哮喘日间、夜间症状评分、哮喘用药评分、总鼻炎症状评分、鼻炎用药评分均显著下降,差异有统计学意义(P0.01);4年疗程组患儿的哮喘药物评分相较1年、2年和3年疗程组更低,差异均有统计学意义(P均0.05);3年组的鼻炎用药评分显著低于2年组(P0.05)。4组患儿的哮喘日间症状评分、哮喘夜间症状评分、总鼻炎症状评分的差异均无统计学意义(P0.05)。结论:通过观察分别接受1~4年不同疗程的SLIT的AS伴AR患儿,发现不同疗程标准化粉尘螨滴剂SLIT对尘螨过敏的患儿均具有显著疗效,且4年疗程疗效最佳。
[Abstract]:Objective: to evaluate the efficacy and safety of sublingual immunotherapy for allergic asthma with rhinitis in children with allergic asthma. Methods: the clinical indexes and data of 158 patients with as with rhinitis who received sublingual immunotherapy with Dermatophagoides farinae were retrospectively analyzed. The patients were divided into 4 groups according to the length of the SLIT course (1, 2, 3 and 4 years) for regular clinical observation and follow-up. The changes of daytime, nocturnal symptom score, asthma medication score, rhinitis symptom score and rhinitis drug use score were evaluated before and after treatment. Adverse reactions during treatment were also recorded. Results: compared with before treatment, the scores of daytime, night symptom, drug use of asthma, total rhinitis and rhinitis were significantly decreased in children with asthma for 1 year, 2 years, 3 years and 4 years of treatment. The score of asthmatic drugs in the 4-year treatment group was lower than that in the 1-year, 2-year and 3-year treatment groups. The score of rhinitis in 3 years group was significantly lower than that in 2 years group (P 0.054.The score of asthma symptom at night and the score of total rhinitis symptom were not statistically significant). Conclusion: by observing the children with AR who received different courses of SLIT for 1 ~ 4 years, it was found that different courses of standardized slit had significant curative effect on children with dust mite hypersensitivity, and the best therapeutic effect was in 4 years.
