散血明目片联合雷珠单抗眼内注射治疗湿性年龄相关性黄斑变性的临床观察

【摘要】：目的观察散血明目片联合雷珠单抗眼内注射治疗湿性年龄相关性黄斑变性(AMD)的疗效。方法将77例(77眼)湿性AMD患者,采用随机原则分为联合治疗组(39眼)和雷珠单抗组(38眼)。联合治疗组:散血明目片口服+雷珠单抗眼内注射;雷珠单抗组:雷珠单抗眼内注射。眼内注射每月1次,连续3次;散血明目片连续口服3月。通过最佳矫正视力,光学相干断层扫描(OCT)及眼底荧光血管造影(FFA)检查,分析治疗前后最佳矫正视力、黄斑视网膜厚度及视网膜渗漏的变化。结果治疗后3个月最佳矫正视力联合治疗组(0.09±0.05 vs 0.38±0.14)、雷珠单抗组(0.08±0.04 vs 0.29±0.15)均有提高,治疗前后差异及治疗后组间差异均有统计学意义(P0.01或P0.05)。治疗后3个月黄斑视网膜厚度联合治疗组[(315.44±21.99)μm vs(237.13±27.14)μm]、雷珠单抗组[(317.33±21.63)μm vs(254.56±26.32)μm]均有减少,治疗前后差异及治疗后组间差异均有统计学意义(P0.01或P0.05)。联合治疗组有28眼渗漏消失,7眼渗漏面积明显缩小,4眼渗漏面积无明显变化;雷珠单抗组24眼渗漏消失,3眼渗漏面积明显缩小,11眼渗漏面积无明显变化,治疗后3个月组间渗漏总改善率差异有统计学意义(P0.05)。结论散血明目片联合眼内注射雷珠单抗治疗湿性AMD可提高视力,减轻黄斑水肿,使视网膜渗漏减轻,是治疗湿性AMD的有效方法。
[Abstract]:Objective to observe the efficacy of Sanxue Mingmu tablet combined with intraocular injection of Lei Zhu McAb in the treatment of wet age-related macular degeneration (AMD). Methods 77 cases (77 eyes) of wet AMD were randomly divided into combined treatment group (39 eyes) and Lei Zhu McAb group (38 eyes). Combined treatment group: Sanxue Mingmu tablet oral Lei Zhu McAb intraocular injection, Lei Zhu McAb group: Lei Zhu McAb intraocular injection. Intraocular injection was done once a month for 3 times, and Sanxue Mingmu tablet was taken orally for 3 months. The best corrected visual acuity (BCVA), macular retinal thickness and retinal leakage were analyzed by means of best corrected visual acuity, optical coherence tomography (Oct) and fundus fluorescein angiography (FFA). Results the best corrected visual acuity (BCVA) combined treatment group (0.09 卤0.05 vs 0.38 卤0.14) and the Lei Zhu McAb group (0.08 卤0.04 vs 0.29 卤0.15) were increased 3 months after treatment. The differences before and after treatment were statistically significant (P0.01 or P0.05). The macular retinal thickness decreased 3 months after treatment in the combined macular retinal thickness group [(315.44 卤21.99) 渭 m vs (237.13 卤27.14 渭 m] and the Lei Zhu McAb group [(317.33 卤21.63) 渭 m vs (254.56 卤26.32 渭 m]. The difference before and after treatment was statistically significant (P0.01 or P0.05). In the combined treatment group, the leakage area of 28 eyes disappeared and 7 eyes significantly reduced the leakage area of 4 eyes, while the leakage area of 3 eyes in 24 eyes of Lei Zhu McAb group decreased significantly, and the leakage area of 11 eyes did not change significantly. Three months after treatment, there was significant difference in total improvement rate of leakage between groups (P0.05). Conclusion Sanxue Mingmu tablet combined with intraocular injection of Lei Zhu McAb in the treatment of wet AMD can improve visual acuity, reduce macular edema and reduce retinal leakage. It is an effective method to treat wet AMD.
