曲伏前列腺素滴眼液对原发性开角型青光眼患者眼压及血浆内皮素-1的影响

【摘要】：目的观察曲伏前列腺素滴眼液对原发性开角型青光眼患者眼压及血浆中内皮素~(-1)的影响。方法 80例原发性开角型青光眼患者随机分为试验组40例和对照组40例。对照组给予马来酸噻吗洛尔滴眼液,每次1滴,早晚各1次。试验组给予曲伏前列腺素滴眼液,每次1滴,睡前1次。2组均长期持续治疗。比较2组治疗前和治疗后3个月的眼压及血浆中内皮素~(-1)的变化情况。结果治疗后,试验组的平均眼压为(17.12±1.80)mm Hg,对照组为(19.02±1.55)mm Hg(P0.05)。治疗后,试验组的前后昼夜眼压差为(3.92±0.89)mm Hg,对照组为(4.55±0.96)mm Hg(P0.05)。治疗后,试验组视网膜中央动脉(CRA)的收缩期峰速(PSV)为(10.79±2.15)cm·s~(-1),舒张末期流速(EDV)为(3.75±0.74)cm·s~(-1),阻力指数为0.61±0.05;对照组的PSV为(8.67±1.77)cm·s~(-1),EDV为(2.71±0.72)cm·s~(-1),阻力指数为0.67±0.12,2组差异有统计学意义(P0.05)。治疗后,试验组睫状后动脉(PCA)的PSV为(13.23±3.16)cm·s~(-1),EDV为(3.75±0.70)cm·s~(-1),阻力指数为0.65±0.07;对照组的PSV为(9.91±2.56)cm·s~(-1),EDV为(2.72±0.73)cm·s~(-1),阻力指数为0.74±0.10,2组差异有统计学意义(P0.05)。治疗后,试验组血浆内皮素~(-1)水平为(45.22±7.56)ng·L~(-1),对照组为(49.32±8.05)ng·L~(-1)(P0.05)。对照组出现眼部不适3例,眼部轻度干涩5例,药物不良反应发生率为20.00%(8/40例);试验组出现睫毛增粗2例,眼红4例,眼部轻度干涩4例,药物不良反应发生率为25.00%(10/40例),差异无统计学意义(P0.05)。结论曲伏前列腺素对原发性开角型青光眼患者的临床疗效良好,可以降低患者的眼压和患者血浆内皮素~(-1)水平。
[Abstract]:Objective to observe the effect of travoprostaglandin eye drops on intraocular pressure and plasma endothelin-1 in patients with primary open angle glaucoma. Methods 80 patients with primary open angle glaucoma were randomly divided into trial group (n = 40) and control group (n = 40). The control group was given tiamolol maleate eye drops, 1 drop each time, 1 time in the morning and evening. The experimental group was treated with travoprostaglandin eye drops, 1 drop each time and 1 time before bedtime. 2 groups were treated continuously for a long time. Intraocular pressure (IOP) and endothelin-1 (et-1) in plasma were compared before and 3 months after treatment in both groups. Results after treatment, the mean intraocular pressure was (17.12 卤1.80) mm Hg in the experimental group and (19.02 卤1.55) mm Hg in the control group (P0.05). After treatment, the intraocular pressure difference was (3.92 卤0.89) mm Hg in the experimental group and (4.55 卤0.96) mm Hg in the control group (P0.05). After treatment, the peak systolic velocity (PSV), end-diastolic velocity (EDV) and resistance index of central retinal artery (CRA) were (10.79 卤2.15) cm s ~ (-1), (3.75 卤0.74) cm s ~ (-1) and (0.61 卤0.05) in the experimental group, and (8.67 卤1.77) cm s ~ (-1) EDV in the control group, respectively (2.71 卤0.72) cm s ~ (-1) and (0.67 卤0.122) cm s ~ (-1), respectively (P0.05). After treatment, PSV of posterior ciliary artery (PCA) was (13.23 卤3.16) cm s ~ (-1) EDV was (3.75 卤0.70) cm s ~ (-1), resistance index was 0.65 卤0.07 in experimental group, and PSV in control group was (9.91 卤2.56) cm s ~ (-1) EDV (2.72 卤0.73) cm s ~ (-1), resistance index was 0.74 卤0.102 (P0.05). After treatment, the level of plasma endothelin 1 was (45.22 卤7.56) ng / L ~ (-1) in the experimental group and (49.32 卤8.05) ng / L ~ (-1) in the control group (P0.05). In the control group, there were 3 cases of ocular discomfort, 5 cases of mild dry eye, the incidence of adverse drug reaction was 20.00% (8 / 40 cases), the test group had 2 cases of eyelash thickening, 4 cases of eye red and 4 cases of mild dry eye. The incidence of adverse drug reactions was 25.00% (10 / 40 cases) with no significant difference (P0.05). Conclusion Travoprostaglandin has a good clinical effect on patients with primary open-angle glaucoma. It can reduce intraocular pressure and plasma endothelin ~ (-1) levels in patients with primary open-angle glaucoma.
