阻塞性睡眠呼吸暂停低通气综合征合并糖尿病患者血清NO和维他命E含量水平的变化

【摘要】:目的:本文通过观察研究对象血清中一氧化氮(NO)和维他命E(VE)水平数值的变化,探讨NO、VE在阻塞性睡眠呼吸暂停低通气综合征并发2型糖尿发生发展过程中的作用。方法:收集2015年1月-2017年2月于山西医科大学第一医院呼吸睡眠监测室、内分泌科室检查及住院的病人群体,按照相关诊断标准分为单纯OSAHS组(OA组),OSAHS合并糖尿病组(OD组),糖尿病组(DM组),正常组(NC组)四组,共计65例,均行多导睡眠监测(PSG),分别记录患者的性别、年龄,测量身高、重量,计算出体重指数(BMI),手动修正睡眠监测报告,并记录每人呼吸暂停指数(AHI)、夜间最低血氧饱和度(LSO2),且于当日行睡眠监测之前抽取肘静脉血,离心后留取血清,采用比色法和微板法分别测定各组血清中VE和NO含量水平,并对四组相关数据进行整理,统计学描写和SPSS软件数据资料分析。结果:1.一般特征比较:四组之间年龄、BMI不存在统计学差别(P0.05)。2.呼吸参数的比较:OA组、OD组两组的AHI、LSO2分别与NC组、DM组存在统计学差别(P值均0.05)。3.血清NO、VE水平测定的结果:与NC组比较,其余三组的VE、NO数值均下降,OD组下降尤为显著,四组间差别存在统计学意义(P0.05)。4.Person相关分析:VE水平数值与AHI呈负相关,与LSO2呈正相关。NO水平数值与AHI呈负相关,与LSO2呈正相关。结论:1.与正常健康人群相比,OSAHS患者NO、VE水平下降,病情愈严重,二者水平下降愈明显,且与AHI成负相关,与LSO2呈正相关。2.糖尿病病人NO、VE较正常健康人群下降,体内存在一定水平的氧化应激。3.OSAHS合并糖尿病组较单一病种组NO、VE水平下降显著,氧化应激水平增强,引发IR,可能是OSAHS合并糖尿病的发生机制之一。
[Abstract]:Objective: to investigate the role of nitric oxide (no) and vitamin E (VE) in the pathogenesis of type 2 diabetes in obstructive sleep apnea hypopnea syndrome (OSAS) by observing the changes of serum nitric oxide (no) and vitamin E (VE) levels. Methods: the patients were collected from January 2015 to February 2017 in the respiratory and sleep monitoring room of the first Hospital of Shanxi Medical University, the Endocrinology Department and the inpatients. According to the diagnostic criteria, 65 patients were divided into four groups: OSAHS group (OD group), diabetes group (DM group), and normal group (NC group). The patients were monitored by polysomnography (PSG), and their sex, age, height and weight were measured. Body mass index (BMI) was calculated, sleep monitoring report was corrected manually, apnea index (AHI) and minimum oxygen saturation (LSO2) were recorded. The levels of VE and no in serum of each group were measured by colorimetric method and microplate method respectively. The related data of four groups were sorted out statistical description and data analysis of SPSS software were carried out. The result is 1: 1. Comparison of general characteristics: there was no statistical difference in BMI between the four groups (P0.05). 2. Comparison of respiratory parameters in the OD group and the control group, there were significant differences between the two groups (P < 0.05). Results: compared with NC group, the values of VENO in the other three groups decreased significantly, and the differences among the four groups were statistically significant (P0.05) .4.Person correlation analysis showed that there was a negative correlation between VENO and AHI in the other three groups. The level of no was negatively correlated with AHI and positively correlated with LSO2. Conclusion 1. Compared with the healthy people, the level of NOVE in OSAHS patients decreased, the more serious the disease, the more obvious the decrease of the two levels, and the negative correlation with AHI, and the positive correlation with LSO2. 2. The level of NOTVE in diabetic patients was significantly lower than that in normal controls. There was a certain level of oxidative stress in the body. 3. The level of NOFVE in OSAHS with diabetes mellitus group was significantly lower than that in the single disease group, and the level of oxidative stress was increased. The initiation of IRI may be one of the mechanisms of OSAHS with diabetes.
