眼周四穴摩灸法联合人工泪液治疗气阴两虚型重症干眼的临床研究

【摘要】：目的:通过观察“眼周四穴摩灸法”联合人工泪液在治疗气阴两虚型重症干眼中所起的疗效,评价该方法治疗气阴两虚型重症干眼的有效性。方法:将90例气阴两虚型重症干眼患者,随机分成A组、B组、C组,每组30例。A组予人工泪液(0.1%玻璃酸钠滴眼液)每日4次点眼治疗,每次一滴。B组予以眼周四穴摩灸法治疗,C组予A组加B组疗法治疗,3组均每日治疗,疗程均为15天。比较治疗前与治疗后的泪液分泌试验(SIT)、泪膜破裂时间(BUT)、角膜荧光素染色评分(FL)及干眼主观症状,评价治疗效果。结果:1.A、B、C三组治疗前及治疗后的泪液分泌值进行比较,组内比较差异均具有统计学意义(P0.05),组间比较C组与B组无统计学差异(P0.05),C组与A组比较有统计学差异(P0.05),B组与A组比较有统计学差异(P0.05)。2.A、B、C三组治疗前及治疗后的泪膜破裂时间进行比较,组内比较差异均具有统计学意义(P0.05)。治疗后组间两两比较均具有统计学差异(P0.05)。3.A、B、C三组治疗前及治疗后的角膜荧光素染色评分进行比较,组内比较差异均具有统计学意义(P0.05)。治疗后组间两两比较均具有统计学意义(P0.05)。4.A、B、C三组治疗前及治疗后的干眼症状评分进行比较,组内比较差异均具有统计学意义(P0.05)。治疗后组间两两比较均具有统计学意义(P0.05)。A、B、C三组治疗后症状缓解的有效率分别为26.7%,83.3%,93.3%。结论:1.在促进泪液分泌方面,眼周四穴摩灸法联合人工泪液优于单纯应用人工泪液点眼;单纯应用眼周四穴摩灸法要优于单纯应用人工泪液者;眼周四穴摩灸法联合人工泪液与单纯应用眼周四穴摩灸法无明显区别。2.在延缓泪膜破裂时间方面,眼周四穴摩灸法联合人工泪液要优于单纯应用眼周四穴摩灸法和单纯应用人工泪液点眼者。单纯应用眼周四穴摩灸法优于单纯应用人工泪液点眼者。3.在角膜荧光素染色评分方面,眼周四穴摩灸法联合人工泪液点眼要优于单纯应用眼周四穴摩灸法和单纯应用人工泪液者。单纯应用眼周四穴摩灸法优于单纯应用人工泪液点眼者。4.在干眼症状评分方面,眼周四穴摩灸法联合人工泪液点眼要优于单纯应用眼周四穴摩灸法和单纯应用人工泪液者。单纯应用眼周四穴摩灸法优于单纯应用人工泪液者。
[Abstract]:Objective: to observe the therapeutic effect of "eye Thursday acupoint moxibustion" combined with artificial tear on dry eyes of severe dry eyes with deficiency of qi and yin, and to evaluate the effectiveness of this method in treating severe dry eyes with deficiency of both qi and yin. Methods: 90 cases of severe dry eye with deficiency of qi and yin were randomly divided into group A and group B, and 30 cases in group A were treated with artificial tear (0.1% sodium hyaluronate eye drops) 4 times a day. Group B was treated with acupoint moxibustion on Thursday, group C was treated with group A and group B, and group 3 were treated daily, the course of treatment was 15 days. Tear secretion test (sit), tear film rupture time (but), corneal fluorescein staining (FL) and dry eye subjective symptoms were compared before and after treatment. Results 1. The tear secretion values before and after treatment were compared among the three groups. There was no statistical difference between group C and group B (P0.05). There was statistical difference between group C and group A (P0.05). There was significant difference between group B and group A (P0.05). 2. Tear film rupture before and after treatment in group A and group B (P0.05). The crack time is compared, The differences were statistically significant (P0.05). After treatment, there was statistical difference between the two groups (P0.05). 3. The corneal fluorescein staining scores before and after treatment were compared among the three groups, and the differences were statistically significant (P0.05). After treatment, there were significant differences between the two groups (P0.05). 4. The scores of dry eye symptoms before and after treatment were compared among the three groups, and the differences were statistically significant (P0.05). After treatment, the effective rate of symptom relief was 26.70.83.3% (P0.05). Conclusion 1. In the aspect of promoting tear secretion, the combination of acupoint moxibustion and artificial tear on eye Thursday is better than that of artificial tear point only, but it is superior to artificial tear moxibustion method only by using eye Thursday point moxibustion method. There was no significant difference between acupuncture and moxibustion combined with artificial tear and simple moxibustion on eye Thursday. 2. In the aspect of delaying the time of tear film rupture, the combination of acupoint moxibustion and artificial tear therapy on eye Thursday is better than that of the eyes only with eye Thursday point moxibustion and artificial tear point. Simple application of eye Thursday point moxibustion is better than using artificial tear point eyes. 3. In the score of corneal fluorescein staining, the combination of acupoint moxibustion and artificial tear point on Thursday was better than that of those who only applied acupoint moxibustion on Thursday and artificial tear only. Simple application of eye Thursday point moxibustion is better than artificial tear point eyes. 4. In the score of dry eye syndrome, the combination of acupoint moxibustion and artificial tear point on eye Thursday was superior to those who only applied acupoint moxibustion on Thursday and artificial tear only. Simple application of eye Thursday point moxibustion was better than that of artificial tear.
