nCPAP治疗对重度OSAHS患者血液流变及同型半胱氨酸水平的影响

【摘要】:目的探讨经鼻持续气道正压通气(nCPAP)治疗对重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血液流变及同型半胱氨酸(Hcy)水平的影响。方法选取经多导睡眠监测仪诊断的32例重度OSAHS患者(观察组),随机选择同期20例经多导睡眠监测仪排除OSAHS的健康者作为对照组。检测观察组患者于nCPAP治疗前及对照组的全血黏度、红细胞聚集指数、红细胞电泳时间等血液流变参数及Hcy水平,进行3个月的nCPAP治疗后,再次检测血液流变参数及Hcy水平,观察nCPAP治疗后OSAHS患者血液流变参数及Hcy水平的变化。结果观察组患者全血黏度、红细胞聚集指数、红细胞电泳时间及血清Hcy水平均高于对照组,差异有统计学意义(P0.05)。经3个月nCPAP治疗后,观察组患者全血黏度、红细胞聚集指数、红细胞电泳时间及血清Hcy水平均较治疗前明显下降,差异有统计学意义(P0.05)。结论重度OSAHS患者存在血液流变异常以及血清Hcy水平升高。nCPAP治疗可改善重度OSAHS患者血液流变异常,降低其血清Hcy水平,可能有助于降低OSAHS并发心血管疾病的风险。
[Abstract]:Objective to investigate the effects of nasal continuous positive airway pressure (nCPAP) on hemorheology and homocysteine (Hcy) levels in patients with severe obstructive sleep apnea hypopnea syndrome (OSAHS). Methods 32 patients with severe OSAHS diagnosed by polysomnography (observation group) and 20 healthy persons excluded from OSAHS by polysomnography were randomly selected as control group. The hemorheological parameters such as whole blood viscosity, erythrocyte aggregation index, erythrocyte electrophoresis time and Hcy level were measured before nCPAP treatment and in control group. After 3 months of nCPAP treatment, the hemorheological parameters and Hcy level were measured again. To observe the changes of hemorheological parameters and Hcy level in OSAHS patients after nCPAP treatment. Results the whole blood viscosity, erythrocyte aggregation index, erythrocyte electrophoresis time and serum Hcy level in the observation group were significantly higher than those in the control group (P0.05). After 3 months of nCPAP treatment, the whole blood viscosity, erythrocyte aggregation index, erythrocyte electrophoresis time and serum Hcy level in the observation group were significantly lower than those before treatment (P0.05). Conclusion Hemorheological abnormality and elevated serum Hcy level in patients with severe OSAHS. NCPAP can improve hemorheological abnormalities and decrease serum Hcy levels in patients with severe OSAHS and may help to reduce the risk of OSAHS complicated with cardiovascular disease.
