儿童人工耳蜗植入后听觉言语康复效果及个体影响因素分析

【摘要】:[目的]探讨儿童人工耳蜗植入后不同阶段的听觉言语康复效果;分析个体因素对人工耳蜗植入后效果的影响。[方法]根据植入年龄、耳聋基因检测结果分别将130例确诊为重度或极重度感音神经性聋患儿分组,采用听觉行为分级标准(CAP)、言语可懂度分级标准(SIR)、婴幼儿有意义听觉整合量表/有意义听觉整合量表(IT-MAIS/MAIS)、有意义言语使用量表(MUSS)对各组植入耳蜗患者0、3、6、12、18、24个月的听觉言语能力发展效果进行评估比较。使用SPSS 19.0统计分析软件和重复测量方差分析的统计方法,对数据进行统计。采用双侧检验,P0.05为差异有统计学意义。[结果]1~3岁组和3~5岁组患儿的预后效果明显优于其他年龄组患儿。GJB2基因相关性耳聋患者的术后效果明显优于其他组患者,而SLC26A4组患者虽然在表面上各量表得分高于未突变患者,但尚缺乏统计学依据。GJB3组患者的预后效果与未突变组患者预后情况基本一致。[结论]①听障儿童人工耳蜗植入后的听觉言语康复效果显著;②植入年龄、耳聋基因是影响听觉言语康复效果的重要因素。
[Abstract]:[objective] to investigate the effects of auditory and verbal rehabilitation in different stages after cochlear implantation in children, and to analyze the influence of individual factors on the effect of cochlear implantation. [methods] 130 children with severe or extremely severe sensorineural hearing loss were divided into two groups according to the age of implantation and the results of gene test. Auditory behavior scale (CAP), speech intelligibility rating Standard (SIR), Infant meaningful auditory Integration scale / meaningful auditory Integration scale (IT-MAIS/MAIS); significant speech use scale (MUSS); The effect of language competence development is evaluated and compared. Using SPSS 19.0 statistical analysis software and the statistical method of repeated measurement ANOVA, the data were statistically analyzed. There was statistical significance in using bilateral test (P0.05). [results] 1 the prognostic effect of 3 years old group and 35 year old group was significantly better than that of other age group. GJB2 gene related deafness patients were better than other group. Although the scores of SLC26A4 patients were higher than those of non-mutation patients on the surface, the prognostic effect of GJB3 group was almost the same as that of non-mutation group. [conclusion] 1 the effect of hearing speech rehabilitation after cochlear implantation in children with hearing impairment is significant and the age of hearing loss gene is an important factor affecting the effect of hearing and speech rehabilitation.
