106例回肠末端溃疡临床分析

【摘要】：研究背景:回肠末端溃疡是回盲部的好发疾病之一,它的发病机制仍未明确,已有研究表明肠道粘膜的异常免疫反应在其发病中发挥着重要的作用;也有人认为是弯曲杆菌感染所致。随着内镜技术的不断进步,该病的检出率正在逐年增加,但由于该部位起病具有隐匿性,起病初期患者的临床症状和体征往往不明显,因此早期诊断较为困难。目的:通过收集2012年5月-2016年4月于我院进行肠镜检查并诊断为回肠末端溃疡的患者,回顾性分析末端回肠溃疡的发病率、年龄分布、性别比及内镜下溃疡特点,增加对回肠末端溃疡性病变的认识。方法:通过收集于我院进行肠镜检查并诊断为回肠末端溃疡的病患106例,系统记录其性别、年龄、内镜下表现及组织病理结果等,然后进行对比研究和统计分析得出结论。结果:总检查人数7452为人,诊断为回肠末端溃疡病变的患者106例,占总检查人数的1.4%。男性59例,占总检查人数的55.7%,女性47例,占总检查人数的44.3%。其中10-19岁4例,占总人数的3.8%;20-29岁6例,占总人数的5.7%;30-39岁24例,占总人数的22.6%;40-49岁22例,占总人数的20.7%;50-59岁21例,占总人数的19.8%;60-69岁19例,占总人数的17.9%;70-79岁7例,占总人数的6.6%;80岁3例,占总人数的2.8%。镜下表现:(1)溃疡数目:多发的溃疡55例,单发的溃疡51例;(2)溃疡形态:类圆形溃疡71例,不规则溃疡13例,口疮样溃疡8例,巨大溃疡性肿物7例,纵形裂隙样溃疡:5例,地图样溃疡2例;(3)伴随情况:单纯溃疡36例,22例伴有回盲瓣炎,16例伴有息肉,14例伴有淋巴滤泡增生,8例伴有结肠炎,4例伴有直肠炎,3例伴有回盲瓣憩室,2例伴有结肠憩室,1例为结肠癌术后。组织病理检查结果:慢性非特异性炎症88例;溃疡性结肠炎6例;结肠癌3例;肠结核3例;高级别内瘤变2例;低级别内瘤变1例;克罗恩病1例;T细胞淋巴瘤1例;白塞病1例。结论:本研究发现回肠末端溃疡的发生率为1.4%左右,30-60岁检出率最多,男女的发病率一般无明显区别;镜下以单发或者多发的类圆形溃疡比较多见,组织病理结果主要为粘膜组织慢性炎,预后较好,少有结肠癌、恶性淋巴瘤等恶性疾病。由于多数病因诊断困难,仍需进一步深入研究。
[Abstract]:Background: ileal ulcer is one of the primary ileocecal disease, its pathogenesis is not clear, studies have shown that the abnormal immune response of intestinal mucosa plays an important role in its pathogenesis; some people think that is caused by Campylobacter infection. With the development of endoscopic technology, the disease detection the rate is increasing year by year, but because of the location of onset with occult onset, clinical symptoms and signs are not obvious, so early diagnosis is difficult. Objective: through collecting the May 2012 -2016 year in April in our hospital were diagnosed as intestinal endoscopy and ileal ulcer were analyzed retrospectively the incidence of ulcer in the terminal ileum rate, age distribution, sex ratio and endoscopic features of ulcer, increase awareness of the ileal ulcer lesions.
